Zusammenfassung

Die juvenile idiopathische Arthritis (JIA) bedarf insbesondere bei schweren Verläufen zur Vermeidung von Folgeschäden und dauerhaften Funktionseinbußen einer konsequenten Therapie. Biologika gewinnen hier zunehmend an Bedeutung. Über deren Wirksamkeit und Sicherheit in der klinischen Routineversorgung informieren die Biologika-Register BiKeR und JuMBO. JuMBO, als Follow-up-Register von BiKeR, liefert aufgrund der Weiterbeobachtung der Patienten im Erwachsenenalter wichtige Informationen zur Langzeitsicherheit von Biologika und zum Langzeit-Outcome der JIA.

Die Daten des JuMBO-Registers bestätigen im Allgemeinen die gute Verträglichkeit der Biologika in der Langzeitanwendung. Schwere unerwünschte Arzneimittelnebenwirkungen werden selten dokumentiert, neu aufgetretene Autoimmunerkrankungen beschränken sich bisher auf wenige Einzelfälle. Ein Schwerpunkt der Sicherheitsanalysen liegt in der Kalkulation der Malignomrate bei DMARD-exponierten JIA-Patienten. Die bisher in JuMBO erfassten Fälle ließen kein erhöhtes Risiko bei Biologika-exponierten im Vergleich zu nur MTX-exponierten Patienten erkennen. Aber mit den bisher 12 gemeldeten Fällen liegt die Malignomrate in JuMBO über der in der Bevölkerung. Aufgrund der geringen Fallzahlen, der teilweisen DMARD-Mehrfachexposition dieser Patienten und der potenziellen Risikoerhöhung durch eine langanhaltende hohe Krankheitsaktivität sind die Daten jedoch mit Vorsicht zu interpretieren. Eine abschließende Beurteilung des Malignomrisikos unter Biologika-Therapie ist mit den vorliegenden Daten nicht möglich. Eine konsequente Nachbeobachtung der jungen Rheumatiker im JuMBO-Register ist unbedingt nötig, um notwendige Informationen zur Therapiesicherheit bei der JIA zu gewinnen.

Summary

Juvenile idiopathic arthritis (JIA) requires consistent therapy, especially in severe cases, to avoid consequential damage and permanent loss of function. Biologics are becoming increasingly important here. The biologic registers BiKeR and JuMBO provide information on their effectiveness and safety in routine clinical care. JuMBO, follow-up register of BiKeR, provides important information on the long-term safety of biologics and the long-term outcome of the JIA through further monitoring of patients in adulthood.

Data from the JuMBO register generally confirm the good tolerability of biologics in long-term use. Serious adverse events are rarely documented, new autoimmune diseases are limited to a few individual cases. Particular attention is paid to the risk of malignant diseases under biologic therapy. The cases recorded so far in JuMBO showed no increased risk in biologic-exposed patients compared to only MTX-exposed patients. But with the 12 cases reported so far, the malignancy rate in JuMBO is higher than in the population. However, the data should be interpreted with caution due to the small number of cases, the multiple DMARD exposure of these patients and the potential risk increase due to prolonged high disease activity. A final assessment of the malignancy risk under biologic therapy is not possible with the available data. A consistent follow-up of the young people with JIA in the JuMBO register is absolutely necessary in order to gain necessary information on drug safety in JIA.
