Zielsetzung Der Aufsatz will aufzeigen, wie die aktuellen Rahmenbedingungen und Entwicklungen
der qualitätsorientierten Steuerung im deutschen Gesundheitswesen zur integrierten
Gesundheitsversorgung durch große Unternehmensverbünde führen.
Methodik Selektive Literaturrecherche und darauf aufbauende deskriptive Modellbildung.
Ergebnisse Die staatliche Krankenhausplanung der Länder verliert trotz der ihr zur Verfügung
gestellten planungsrelevanten Qualitätsindikatoren an Einfluss zugunsten intermediärer
Steuerungsinstanzen wie dem G-BA, der den Markt zunehmend reguliert. Es wird zu einem
weiteren Strukturwandel der Leistungserbringer-Landschaft durch qualitätsorientierte
Versorgungsgestaltung (Zentrumsbildung durch Mindestfallzahlen) kommen.
Schlussfolgerung Die Reduzierung der Versorgungskapazitäten dürfte die staatliche Sicherstellungsproblematik
zusätzlich verschärfen. Unternehmensverbünde könnten diese Entwicklung abfedern und
zu einer wirtschaftlichen sowie qualitativ hochwertigen Versorgung führen.
Abstract
Objective/goal setting The paper aims to show how the current framework conditions and developments in quality-oriented
management in the German health care system are leading to integrated health care
provision by large corporate networks.
Methodology Selective literature research and descriptive modelling based on it.
Results In spite of the provided planning relevant quality indicators, state hospital planning
in the Länder is losing influence in favour of intermediary control bodies such as
the G-BA, which is increasingly regulating the market. There will be a further structural
change in the service provider landscape through quality-oriented care organisation
(centre formation through a minimum number of cases).
Conclusion The reduction in supply capacities is likely to further exacerbate the problem of
state guarantees. Business alliances could cushion this development and lead to an
economic and high-quality health care organisation.
