Zusammenfassung

Die Gicht ist die häufigste metabolische Arthropathie. Obwohl Ätiologie und Pathogenese des Krankheitsbildes gut untersucht sind, bereitet die Diagnosesicherung mittels des Nachweises von Natriumuratkristallen aus einem Gelenkpunktat oder anderen betroffenen Geweben mitunter große Schwierigkeiten. Wir berichten von einem außergewöhnlich schweren Fall einer generalisierten Gichtarthropathie mit atypischer Manifestation und kompliziertem Verlauf. Dabei sollen wichtige Differenzialdiagnosen ausgedehnter ossärer Destruktionen beleuchtet werden.

Abstract

Gout is the most common metabolic arthropathy. Although its aetiology and pathogenesis are well understood, it is sometimes very difficult to confirm the diagnosis by detecting sodium urate crystals from aspirates of joints or other affected tissues. We report an exceptionally severe case of generalised gout arthropathy with atypical manifestation and a complicated course. Differential diagnoses of extensive osseous destruction are discussed as well.
