Percutaneous pinning is the gold standard in the treatment of pediatric supracondylar humerus fractures (SHF). Crossed pin configuration provides superior biomechanical stability in certain fracture patterns compared to other pin configurations. However, the medial pin increases the risk of iatrogenic ulnar nerve injury to a rate of 4.1 % due to a pin-nerve conflict, which has inspired surgeons to develop safer alternative approaches (Babal et al. J Pediatr Orthop 2010; 30: 253–263).
