Management bei multiresistenten Erregern

In vielen Bereichen, nicht nur in der Intensivmedizin, ist die Zunahme der Antibiotikaresistenz ein großes Problem. Hier trägt jeder Behandler bei der Verschreibung von Antibiotika eine Mitverantwortung für die lokale Resistenzlage. Der Beitrag fasst das Vorgehen im Rahmen der Behandlung sowie der Prävention der Besiedlung mit multirestenten Erregern zusammen.

Kernaussagen
  • Die Kolonisation mit multiresistenten Erregern ist für betroffene Patienten ungünstig, da im Falle einer Infektion weniger Antibiotika zur Verfügung stehen und die empirische Therapie öfter inadäquat ist.

  • Die Ausbreitung multiresistenter Erreger wird durch krankenhaushygienische Maßnahmen und einen rationalen Einsatz von Antibiotika verringert.

  • In Deutschland gibt die Kommission für Krankenhaushygiene und Infektionsprävention am Robert Koch-Institut (KRINKO) Empfehlungen zum Umgang mit multiresistenten Erregern, die auf das jeweilige Krankenhaus angepasst werden müssen.

  • Neben der Basishygiene kommen bei vielen multiresistenten Erregern auch Barrieremaßnahmen (Einzelzimmerisolierung und Anwendung von persönlicher Schutzausrüstung) zum Einsatz.

  • In den letzten Jahren wurden einige neue Antibiotika zugelassen, die insbesondere im gramnegativen Spektrum eine Erweiterung der Therapiemöglichkeiten bieten. Auch für diese neuen Antibiotika wird bereits eine Resistenzentwicklung beobachtet.

  • Der Einsatz neuer Antibiotika erfordert eine detaillierte Bewertung sowohl der lokalen Resistenzsituation als auch der individuellen Sensibilitätstestung.
