Chronic multidirectional instability of the interphalangeal joint (IPJ) of the thumb is mostly treated by arthrodesis of the joint. We report about two patients where we reconstructed both, the radial and the ulnar collateral ligament of the IPJ of the thumb with use of one tendon graft. The technique is similar to the technique described by van Riet et al for simultaneous reconstruction of the medial and lateral elbow ligament in chronic multidirectional ligamentous instability using one circumferential tendon graft [1]. Ligament reconstruction is not indicated in the presence of IPJ arthrosis, deformity, or inflammatory diseases, such as rheumatoid arthritis.
