Die Kodierung chronischer Schmerzen in der ICD-11
The coding of chronic pain in the ICD-11ZUSAMMENFASSUNG
Mit der 11. Revision enthält die Internationale Klassifikation der Krankheiten und verwandter Gesundheitsprobleme (ICD-11) erstmalig ein eigenes Kapitel, in dem Diagnosen chronischer Schmerzen systematisiert sind. Chronische Schmerzen sind Schmerzen, die mehr als 3 Monate andauern oder wiederkehren. Alle Diagnosen in diesem Kapitel sind einheitlich als biopsychosozial konzipiert. Die Klassifikation enthält 7 Hauptkategorien. Eine dieser Kategorien enthält chronische primäre Schmerzen. Bei chronischen primären Schmerzen werden die Schmerzen selbst als eine eigenständige Krankheit aufgefasst. 6 Kategorien chronischer sekundärer Schmerzen bilden Schmerzen ab, die Symptom einer anderen Erkrankung sind. Zu den chronischen sekundären Schmerzen zählen chronische Schmerzen durch eine Krebserkrankung oder -behandlung, chronische Schmerzen nach Operationen oder Unfällen, chronische neuropathische Schmerzen und chronische sekundäre Kopfschmerzen und orofaziale, viszerale und muskuloskelettale Schmerzen. Für chronische primäre und sekundäre Schmerzen stehen optionale Zusatzcodes zur Erfassung behandlungsrelevanter schmerzbezogener Parameter zur Verfügung.
ABSTRACT
With its 11th revision, the International Classification of Diseases and Related Health Problems (ICD-11) for the first time contains a separate and systematic chapter of chronic pain diagnoses. Chronic pain is pain that lasts or recurs for more than 3 months. All diagnoses in this chapter are based on a biopsychosocial model of pain. The classification contains 7 main categories. The first category is chronic primary pain. In the case of chronic primary pain, the pain itself is regarded as a disease and an independent health problem. Further 6 categories of chronic secondary pain represent pain that is a symptom of another disease. Chronic secondary pain includes chronic pain due to cancer or its treatment, chronic pain after surgery or accidents, chronic neuropathic pain and chronic secondary headaches and orofacial, visceral, and musculoskeletal pain. For chronic primary and secondary pain, optional additional codes are available that permit to record pain-related parameters relevant to the treatment.
