Progressive multifokale Leukoenzephalopathie: eine wichtige Differenzialdiagnose einer Läsion der weißen Substanz bei immunsupprimierten Patienten

Einleitung

Die progressive multifokale Leukoenzephalopathie (PML) ist eine seltene, opportunistische Infektion des Gehirns, die durch das JC-Virus verursacht wird. Das JC-Virus wird wahrscheinlich oral übertragen und führt bei Immunkompetenzen zu einer asymptomatischen Infektion. Bei immunsupprimierten Patienten, beispielsweise im Rahmen von HIV-Infektionen oder hämatologischen Erkrankungen, kommt es zu einer Reaktivierung der Erkrankung. Aufgrund der zunehmenden Verbreitung immunsuppressiver beziehungsweise immunmodulatorischer Therapien nimmt die Prävalenz der Erkrankung zu [1] [2]. Die Diagnosestellung erfolgt mittels charakteristischer MRT-Bildgebung sowie Virusnachweis im Liquor. Wir berichten über einen Patienten mit PML, der im Rahmen einer immunsuppressiven Therapie aufgrund eines Lymphoms erkrankte.
