Biologisches Kariesmanagement in der Kinderzahnheilkunde – Teil 2

Bei der minimalinvasiven Kinderzahnmedizin kommen ganzheitliche Maßnahmen mit Prävention, Identifizierung von Risikofaktoren, biologischem und minimalinvasivem Kariesmanagement bis hin zum Recall zur Kontrolle des kariösen Prozesses und zur Förderung der Zahngesundheit zum Einsatz. Auf einzelne Managementstrategien für kariöse Milchzähne und ihre Ergebnisse wird nun in Teil 2 dieses Beitrags detaillierter eingegangen.

Das Konzept der minimalinvasiven Zahnmedizin beinhaltet neben kariespräventiven Maßnahmen „biologische“ und minimalinvasive operative Kariesmanagementstrategien, die einen möglichst großen Erhalt gesunder und remineralisierbarer Zahnhartsubstanzen anstreben.

Bei Milchzähnen kommt neben dem lokalen Biofilmmanagement auch die Versiegelung von Biofilm bzw. kariösen Läsionen zum Einsatz.

Bei Anwendung der Hall-Technik wurden im Vergleich zur konventionellen Füllungstherapie vorteilhafte Ergebnisse berichtet.

Daneben kommen Techniken der atraumatischen restaurativen Therapie, bevorzugt bei 1-flächigen okklusalen Restaurationen, zum Einsatz.

Bei der selektiven Kariesexkavation entscheidet die Tiefe der kariösen Läsion über den Endpunkt der Kariesexkavation.

Es bedarf einer einheitlichen Aus- und Weiterbildung von Zahnärzten hinsichtlich aktueller Kariesmanagementstrategien und einer angemessenen Vergütung, damit dieses ganzheitliche Konzept zur Verbesserung und zum Erhalt der oralen Gesundheit von Kindern Einzug in die Praxis finden kann.
