Zusammenfassung

Hintergrund

Es existieren keine klaren Empfehlungen zur enteralen Kontrastmittel (KM)-Applikation im Rahmen abdomineller Computertomografie (CT)-Untersuchungen. Eine vorausgegangene Umfrage unter Mitgliedern der Deutschen Röntgengesellschaft (DRG) identifizierte eine sehr heterogene und indikationsabhängige Vorgehensweise im klinischen Alltag. Ziel der vorliegenden Arbeit ist die Erstellung entsprechender Handlungsempfehlungen.

Material und Methoden

Handlungsempfehlungen zur enteralen Kontrastmittel-Applikation wurden durch Mitglieder der AG Abdomen der Deutschen Röntgengesellschaft (DRG) erarbeitet. Die Methodik orientierte sich am Vorgehen einer S1-Leitlinie mit informellem Expertenkonsens. Klinisch relevante Fragestellungen wurden systematisch identifiziert, thematisch strukturiert und unter Berücksichtigung der aktuellen Evidenz in der Literatur und ggf. vorhandener Leitlinien bewertet.

Ergebnisse und Schlussfolgerung

Es gibt einzelne Szenarien, in denen die enterale KM-Gabe indiziert ist, entsprechende Handlungsempfehlungen haben wir im Sinne einer good clinical practice (GCP) ausgearbeitet. Dabei ist die Applikation insbesondere positiver enteraler KM im Rahmen abdomineller CT-Untersuchungen für viele klinische Fragestellungen mittlerweile obsolet.

Kernaussagen

Bei vielen Fragestellungen kann auf eine enterale KM-Gabe verzichtet werden,
bei Tumorerkrankungen des Ösophagus und Magens sollte eine enterale Kontrastierung mittels „Hydro-Technik“ erfolgen,
positive enterale KM werden primär zur Abklärung gastrointestinaler Fisteln und Anastomosen empfohlen.
