Abstract
Aneurysm of the Carotid Artery in a child. Successful therapy.
This is a case report of a saccular aneurysm of the carotid artery in a 10 year old
child. Attention should be called to the rarity of this case. A subarachnoid hemorrage
in this child led to a serious condition with coma, hemiplegia, and autonomic system
disturbances. Especially remarkable was an extreme spasm of the carotid artery and
hydrocephalus which was recognizable angio-graphically along the course of the pericallosal
artery. Both findings in this form are rarely seen in adults. The remarkably good
recovery from the disturbances caused by the aneurysmal hemorrhage is described. The
hemiplegia almost completely resolved, a process which in accordance with other authors,
is related to the capability of compensation and the elasticity of the cerebral circulation
in the child.
Zusammenfassung
Es wird über ein säckchenförmiges Aneurysma der Carotisgabel bei einem 10jährigen
Mädchen berichtet. Die Subar& hniidalblutung hatte bei dem Kind zu einem schweren
Krankheitsbild mit Bewußtlosigkeit, Halbseitenlähmung und vegetativen Störungen geführt.
Auffällig war ein extremer Spasmus der A. carotis und ein Hydrocephalus, der insbesondere
angiographisch am Verlauf der A. pericallosa arkennbar war — beides Veränderungen,
die in dieser Form bei Erwachsenen kaum gesehen werden. Es wird hingewiesen auf die
ausgezeichnete Restitution der durch die Aneurysmablutung bewirkten Störungen: Ein
Mittelhirnsyndrom mit Hemiplegie bildete sich fast völlig zurück, ein Vorgang der
in Übereinstimmung mit anderen Autoren mit der Elastizität und Kompensationsfähigkeit
des kindlichen cerebralen Kreislaufes in Zusammenhang gebracht wird.
