Cor triatriatum sinistrum biventriculare

Zusammenfassung

Obwohl es sich bei Cor triatriatum um eine sehr seltene Herzmißbildung handelt, sind in den letzten Jahren 3 Fälle beschrieben worden, die zur Operation kamen. Bei keinem wurde die Diagnose prae- und peroperativ gestellt. Der vorliegende 4. Fall wurde ebenfalls unter einer falschen Diagnose (Vorhofseptumdefekt) operiert. Es wird deshalb versucht, das klinische Bild, die Differentialdiagnose und die chirurgisch-anatomischen Besonderheiten dieser Mißbildung herauszuarbeiten. Ebenso wird auf Grund der pathologisch-anatomischen Literatur die noch problematische Entwicklungsgeschichte des Cor triatriatum besprochen.

Summary

Cor triatriatum is a very rare malformation of the heart, but in the last few years 3 cases have been reported that were operated upon. In neither case was the correct diagnosis established pre-or intraoperatively. The case presented was also misdiagnosed as auricular septal defect. The correct diagnosis was made at autopsy. An attempt has been made to point out the clinical picture, differential diagnosis and anatomical details of this malformation. The literature was reviewed with special regard to the various conflicting developmental theories.

Résumé

Bien que le Cor triatriatum soit une malformation cardiaque très rare, on en a décrit 3 cas ees dernières années, qui furent opérés. Dans aueun des cas on n'avait posé le diagnostic pré et per-opératoire. Le 4^e cas décrit ci-dessus aussi faussement diagnostiqué (défect du septum interauriculaire) et opéré. C'est pourquoi on s'est efforcé d'en préciser l'image clinique, le diagnostic diíférentiel et l'aspect anatomique chirurgical. Aprés une étude de son ana tomo-pathologie dans la littérature, on examine l'histoire de son évolution encore problématique.
