Chirurgische Probleme der transvenösen, intrakardialen Schrittmacherimplantation

Zusammenfassung

An 323 Patienten mit transvenöser intrakardialer Schrittmacherimplantation sind in den Jahren 1968 bis 1973 489 operative Eingriffe vorgenommen worden. Die Gesamtkomplikationsrate liegt bei 25% aller operativen Eingriffe. Von den 119 Korrektureingriffen erfolgten 41 (34%) wegen einer Elektrodendislokation, 38 (33%) wegen eines Elektrodenbruches und 13 (11%) wegen eines Elektrodenwackelkontakts, 18 (14%) wegen entzündlicher Reaktionen an Elektrode und Schrittmacher. Die operativen und technischen Möglichkeiten zur Verhütung der Komplikationen werden aufgezeigt. Gegenüber anderen Methoden wird der transvenösen intrakardialen Implantation der Vorzug gegeben.

Summary

489 Operations were performed at 323 patients with transvenous implantation of cardiac pacemarkers in 1968–1973. The average of the complications is coming to 25 p.c. of all operations. 41 (34 p.c.) operations of correction are caused by a dislocation of the electrode, 39 (33 p.c.) by a break of the electrode, 18 (14 p.c.) by infectious complications. The operative and technical possibilities to prevent the complications are described. The method of transvenous implantation of cardiac pacemakers is preferred.
