Zusammenfassung

Die chirurgischen Ergebnisse zweier hinsichtlich Klinik und Katheterdaten ähnlicher Patientengruppen werden verglichen, bei denen ein Aortenklappenersatz in Magnesiumaspartat-Kardioplegie kombiniert mit milder (Gruppe A = 71 Patienten) bzw. tiefer Hypothermie (Gruppe B = 69 Patienten) vorgenommen wurde. Die Operationsletalität betrug in Gruppe A 5,6%, in Gruppe B 4,3%. Zwei Patienten in Gruppe A verstarben im low cardiac output, drei Patienten benötigten inotrope Kreislaufunterstützung. In Gruppe B beobachteten wir keine myogene Herzinsuffizienz, kein Patient benötigte Sympathomimetika. Rhythmusstörungen traten in beiden Gruppen bei über 30% der Patienten auf. Tiefe Körperhypothermie (Ösophagustemperatur 25°C) und hypotherme Injektionskardioplegie mit Magnesiumaspartat bieten guten Myokardschutz bis zu einer Anoxiezeit von 60 Minuten.

Summary

The surgical results of two similar groups of patients are compared in whom aortic valve replacement was performed utilizing magnesium-aspartate-cardioplegia combined with mild (group A = 71 patients) and profound (group B = 69 patients) hypothermia, respectively. Mortality was 5.6% in group A and 4.3% in group B. Two deaths in group A were due to a severe low cardiac output syndrome, three patients required continuous inotropic circulatory support during the early post-operative course. There was no low cardiac output in group B, no patient required inotropic support. Profound body cooling (esophageal temperature 25°C) and hypothermic injection cardioplegia with magnesium-aspartate provide efficient myocardial protection up to 60 minutes of anoxia.
