Zusammenfassung

Als praktisch verwertbare Dignitätsdiagonse unterschiedlicher Mammakarzinome und ihrer Entwicklungsstadien wird das auf der Nomenklatur der IUAC aufbauende Stockholmer Schema aufgrund der epikritischen Zehnjahresstudie von Hultborn und Törnberg empfohlen. Die primäre Strahlenreaktion bei 92 Mammakarzinomen wurde auf der Basis dieses Einteilungsprinzips analysiert. Der unterschiedliche Ent-differenzierungsgrad des Tumors zur Zeit der Probe-exzision zeigte keine Parallelen zur Anamnesedauer oder zur „Verschleppungszeit”, er ist vielmehr Ausdruck eines biologisch differenten Verhaltens des jeweiligen Tumors. Strahlenempfindlichkeit und -resistenz weisen weitgehende Parallelen zum Malignitätsgrad des Mammakarzinoms auf. Mittels hochdosierter Telekobaltbestrahlung sind bei einem Teil der Mammakarzinome (Malignitätsgrad 1 und 2) erhebliche gewebskaustische Effekte zu erzielen, dabei werden jedoch auch besondere Abwehrreaktionen des Wirtsgewebes zerstört. Histologisch nachweisbare regressive Veränderungen sind zeitlich limitiert; einige Beobachtungen sprechen für eine (individuell variable?) kritische Zeitgrenze von drei bis sechs Wochen nach Beendigung der Strahlenbehandlung. Die Strahlenreaktion von Karzinommetastasen ist geringer als die des Primärtumors; eine vermehrte Absiedlungsmöglichkeit von Mammakarzinomen während und nach Bestrahlung sowie das bevorzugte Angehen in prophylaktisch vorbestrahlten paramammären Regionen muß aufgrund einschlägiger Beobachtungen bei Tier und Mensch einer kritischen Prüfung unterzogen werden. Ob axilläre Metastasen vorliegen, ist klinisch nicht sicher zu entscheiden; der Verzicht auf die radikale Mastektomie als Mittel der Wahl scheint daher nicht ratsam.

Summary

The Stockholm Scheme, based on the nomenclature of the IUAC, as derived from the ten-year study of Hultborn and Törnberg, is recommended as an, in practice, prognostically useful classification of different types of carcinoma of the breast and their stages. The primary reaction to radiation was analysed on this basis in 92 cases of carcinoma of the breast. There was no correlation between different degrees of differentiation of the tumour cells at the time of biopsy and the period elapsed between first signs of symptoms and treatment (delay period), but rather it was the expression of biologically different behaviour of the individual tumours. Sensitivity and resistance to radiation went parallel with the degree of malignancy. Using high doses of telecobalt irradiation, considerable tissue effects were obtained in some of the breast carcinomas (malignancy Grade I and II), but some of the defence reactions of the host tissue were also destroyed at the same time. Regressive changes, which could be histologically demonstrated, were limited as to time: personal observations suggest the existence of an (individually variable?) critical time limit of three to six weeks after the end of radiotherapy. Radiation reaction of metastases is less than that of the primary tumour: the possibility of an increased metastasization of carcinoma of the breast during or after radiotherapy, as well as any preferential spread to prophylactically irradiated paramammary regions need to be subjected to critical analysis in view of observations in both animal and man. Whether axillary métastases are present cannot be determined with certainty by clinical examination: deliberately to avoid radical mastectomy is thus not recommended.

Resumen

Problemas terapéuticos del carcinoma de mama referidos a la radiorreacción primaria y letalidad

Como diagnóstico práctico y utilizable de malignidad de los diversos carcinomas de mama y sus períodos de desarrollo se recomienda el esquema de Estocolmo, basado en la nomenclatura de la IUAC, debido a los estudios epicríticos, de 10 años de duración, de Hultborn y Törnberg. La primera radiorreacción en 92 carcinomas de mama fue analizada sobre la base de este principio de clasificación. El distinto grado de indiferenciación del tumor en el momento de la punción biópsica no mostró ningún paralelismo a la duración de la anamnesis o al “tiempo de propagación”, él es más bien expresión del diferente comportamiento biológico de cada tumor. Radiosensibilidad y radiorresistencia señalan el gran paralelismo al grado de malignidad del carcinoma de mama. Mediante dosificación alta de irradiación de telecobalto se pueden conseguir considerables efectos cáusticos tisulares en una parte de los carcinomas de mama (grado de malignidad 1 y 2), no obstante, también se destruyen aquí las reacciones especiales de defensa del tejido receptor. Alteraciones regresivas, histológicamente comprobables, están limitadas temporalmente; algunas observaciones hablan a favor de un límite crítico de tiempo (¿Individualment variable?) de 3 a 6 semanas tras la terminación de la radiación. La radiorreacción de las metástasis carcino-matosas es más pequeña que la del tumor primario; una mayor posibilidad de dispersión del carcinoma de mama durante y tras la irradiación así como el prendimiento selectivo en regiones paramamarias profilácticamente radiadas con anterioridad tiene que ser sometida a una prueba crítica, debido a las oportunas observaciones en animales y seres humanos. Clínicamente no se puede decidir con seguridad, si hay metástasis axilares; la renuncia a la mastectomía radical como método de elección no parece, por consiguiente, aconsejable.
