Zusammenfassung

Der Nachweis von N-Acetyl-p-aminophenol (= p-Aminophenol) mit der von uns leicht modifizierten Methode nach Welch und Conney bei Einnahme oder Mißbrauch phenacetinhaltiger Analgetika hat sich uns bei mehr als 6000 Untersuchungen an Spontanurin im täglichen Gebrauch bewährt. Extinktionswerte über 0,056 o. D. (620 mµ) wurden bei gesunden Frauen bis zu 10% und bei Männern bis zu 5% in Fabrikpopulationen gemessen; sie bestätigen die Einnahme von Phenacetin oder p-Aminophenol. Bei Verwendung von Spontanurin wird nur etwa die Hälfte derjenigen Personen erfaßt, die phenacetinhaltige Analgetika eingenommen haben, bei Verwendung von 24-Stunden-Urin praktisch alle. Mehrmaliger Nachweis erhöhter p-Aminophenol-Konzentrationen im Urin ist ein starker Hinweis auf eine mißbräuchliche Einnahme phenacetinhaltiger Analgetika, was anhand von Beispielen dargelegt wird. Belastungsuntersuchungen mit p-Aminophenol und Phenacetin ergeben für Leberkranke eine Ausscheidung von p-Aminophenol im 24-Stunden-Urin wie beim Gesunden; von Patienten mit Niereninsuffizienz nach iangjährigem Mißbrauch phenacetinhaltiger Analgetika wird dagegen regelmäßig weniger p-Aminophenol im Urin ausgeschieden.

Summary

A simple method for assaying N-acetyl-p-aminophenol (APAP), slightly modified from that described by Welch and Conney, has proved valuable in the detection of the intake, in normal or excessive amounts, of phenacetin-containing analgesics. Extinction values of 0.056 optical density (at 620 mµ) were obtained in up to 10% of healthy women and 5% of healthy men among a factory staff, demonstrating that they had taken phenacetin or APAP. If only spontaneously voided specimens of urine are measured, only about 50% of persons taking phenacetin-containing analgesics will give a positive test, while testing 24-hour urine gives practically 100% positive results after phenacetin intake. If increased APAP concentrations in urine are discovered on repeated specimens, there is strong indication of an excess intake of phenacetin-containing analgesics. Balance studies with APAP and phenacetin in patients with liver damage showed similar amounts of APAP being excreted in 24-hour urine as in healthy persons, while in patients with renal insufficiency who had taken excessive amounts of phenacetin-containing analgesics over many years excretion was found to be decreased.

Determinación de p-aminofenol en orina como método rutinario para la comprobación de la ingestión de fenacetina

La comprobación de N-acetil-p-aminofenol (= p-aminofenol) con el método según Welch y Conney, ligeramente modificado por nosotros, en los casos de ingesta o abuso de analgésicos que contienen fenacetina se ha acreditado ante nosotros en el uso diario, en más de 6000 análisis de orina espontánea. Valores de extinción por encima de 0,056 o. D. (620 mµ) fueron medidos en mujeres sanas hasta un 10% y en hombres sanos hasta un 5% de la población industrial; estos valores confirman la ingestión de fenacetina o p-aminofenol. Empleando orina espontánea sólo se abarca cerca de la mitad de las personas que han tomado analgésicos que contienen fenacetina y empleando la de 24 horas se abarcan prácticamente todas ellas. Una repetida comprobación de concentraciones elevadas de p-aminofenol en orina es un fuerte indicio sobre una ingestión abusiva de analgésicos que contienen fenacetina, lo que se expone a base de ejemplos. Análisis de sobrecarga con p-aminofenol y fenacetina mostraron, para enfermos hepáticos, una elevación de p-aminofenol en las 24 horas igual a la de los sanos, por el contrario se elimina regularmente menos p-aminofenol en orina en los pacientes con insuficiencia renal consecutiva a un abuso, de varios años de duración, de analgésicos que contienen fenacetina.
