Zusammenfassung
Von 219 operierten Patienten wurden die präoperativ mit Dimer-X® (Dimeglumin-iocarmat) gewonnenen Myelogramme ohne Kenntnis des Operationsbefundes erneut interpretiert. Dabei ergab sich in 77% eine Übereinstimmung zwischen myelographischer Deutung und chirurgischer Diagnose. Als schwierig und unsicher erwies sich die Interpretation, wenn nur ein Röntgensymptom vorhanden war. Zur Ausschöpfung des Informationsgehaltes der Untersuchung sollten deshalb auch die anamnestischen und klinischen Befunde gleichwertig herangezogen werden. Nur in klinisch unklaren Fällen hat die Myelographie ihre Berechtigung; daher ist, auch wegen der Möglichkeit von Komplikationen, eine strenge Indikationsstellung erforderlich.
Summary
The pre-operative myelograms performed using Dimer-X (dimeglumine-iocarmate) of 219 patients were re-evaluated post-operatively without knowledge of the operative findings. In 77% the myelographic interpretation was in agreement with the surgical diagnosis. The interpretation was difficult and unreliable when only one or few radiographic signs were present. In order to achieve optimal benefit from the investigation the history and clinical findings should also be considered as equally important. Myelography is only justified in clinically unclear cases. In view of this and possible complications a stringent indication is necessary.
