Der Einfluß von Tolbutamid auf die diabetische Fettleber^*

Zusammenfassung

Die Wirkung von Tolbutamid auf die Fettleber von Patientinnen mit leichtem Altersdiabetes wurde im Doppelblindversuch geprüft. Bei quantitativer Bestimmung des Triglyceridgehaltes im Leberpunktat ergab sich nach zweimonatiger Behandlungszeit kein Unterschied zwischen Tolbutamid und Placebo. Während Cholesterin, freie Fettsäuren und Phospho-lipoide im Serum durch Tolbutamid nicht signifikant verändert wurden, bewirkte es einen signifikanten Abfall des Blutzuckers und einen signifikanten An- stieg des Leberglykogens und der Serumtrigly-ceride. Die histologische Schätzung des Leberfettgehaltes erscheint zur statistischen Beurteilung lipotro-per Effekte ungeeignet. Die geschätzten Werte weichen von den chemisch ermittelten häufig und beträchtlich ab.

Summary

In a double-blind test the effect of tolbutamide on fatty liver in diabetic women (mild diabetes of the elderly) was studied (eight received tolbutamide, seven placebo). After a treatment period of two months triglyceride content of a liver biopsy (by needle puncture) revealed no significant difference between the patients who had received tolbutamide and those on a placebo. While cholesterol, free fatty acid and phospholipids in serum were not significantly changed by tolbutamide administration, the drug did cause a significant fall in blood sugar and a significant rise in liver glycogen and serum-triglycerides. The histological estimation of fat content in liver cells is unsatisfactory for the statistical evaluation of lipotropic effects. The estimated effect differs frequently and significantly from that chemically determined.

La influencia de la tolbutamida en el hígado graso diabético

El efecto de la tolbutamida sobre la degeneración grasa de hígado de pacientes con diabetes antigua y leve fue examinado en ensayo de doble-ciego. En las determinaciones cuantitativas de los triglicéridos en la punción hepática no se demostró, después de dos meses de tratamiento, ninguna diferencia entre la tolbutamida y el placebo. Mientras que la colesterina, los ácidos grasos libres y los fosfolípidos en el suero no se alteraron significativamente por la tolbutamida, se produjo und significativo descenso del azúcar en sangre y un aumento marcado del glucógeno hepático y de los triglicéridos séricos. La valoración histológica del contenido graso del hígado no parece adecuada para la interpretación estadística de los efectos lipotróficos. Los valores así considerados divergen con frecuencia y en importancia de los determinados químicamente.
