Zusammenfassung

27 dauerantikoagulierte Patienten mußten wegen einer Blutung im Zentralnervensystem operiert werden. Diese Komplikationen traten vorwiegend bei Männern auf und endeten in fast der Hälfte der Fälle entweder mit dem Tod (n = 9) oder mit einer schweren Invalidität (n = 4). Weniger als ein Viertel der Patienten (n = 6) hatte bei Spitaleintritt einen Prothrombinwert im gewünschten therapeutischen Bereich. Hauptsächlich wurden ältere und alte Patienten betroffen, Hypertoniker waren kaum vertreten. 22 Hämatome lagen intrakraniell, fünf im Spinalkanal. Mehr als die Hälfte der Blutungen war subdural (n = 14), häufig doppelseitig (n = 8), lokalisiert, sie hatten meist einen subakuten bis chronischen Verlauf. Erstmals beobachtet wurde ein intrazerebelläres Hämatom; es wies einen protrahierten Verlauf auf. Die Therapie der Antikoagulantienhämatome besteht in operativer Ausräumung des Blutergusses. Die Prognose ist je nach Lokalisation unterschiedlich. Insgesamt starben neun Patienten: sechs von sieben mit intrazerebralen Blutungen, zwei von 14 mit subduralen Blutungen und einer von fünf Patienten mit einer Blutung im Spinalkanal.

Summary

Operations on the central nervous system (CNS) because of bleeding were necessary in 27 patients on permanent anticoagulant treatment. Men predominated and the bleeding proved fatal in almost half the cases (9) or ended in severe invalidism (4). Less than a quarter of the patients (6) had had a prothrombin value within the desired therapeutic range on entering hospital. Most of the patients were elderly or old. There were no hypertensives in this group. Intracranial haematoma was present in 22, spinal-canal bleeding in five. More than half of the bleeding episodes were subdural (14), frequently bilateral (8), and usually took a subacute to chronic course. There was one case of intracerebellar haematoma, with a protracted course. Treatment of CNS haematoma on anticoagulant therapy consisted of operative evacuation of the haematoma. The prognosis depended on the localization. Six of the seven patients with intracerebral bleeding died, two of 14 with subdural bleeding and one of five with spinal-canal bleeding.

Aspectos neuroquirúrgicos de las hemorragias por anticoagulantes en el sistema nervioso central

27 pacientes sometidos a una anticoagulación permanente tuvieron que ser operados a consecuencia de una hemorragia en el sistema nervioso central. Estas complicaciones se presentaron sobre todo en hombres, terminando en casi la mitad de los casos por fallecimiento (n = 9) o con una invalidez grave (n = 4). Menos de un cuarto de los pacientes (n = 6) tenía a su ingreso en la clínica un valor de protrombina en el índice terapéutico deseado. Preferentemente estaban afectados los pacientes de edad avanzada. Apenas había hipertensos. 22 hematomas tenían una localización intracraneal y cinco se hallaban en el conducto raquídeo. Más de la mitad de las hemorragias eran de localización subdural (n = 14), frecuentemente bilaterales (n = 8), teniendo la mayoría de ellas una evolución subaguda a crónica. Por primera vez se observó un hematoma intracerebelar, mostrando una evolución prolongada. La terapéutica de los hematomas por anticoagulantes consiste en la evacuación quirúrgica del derrame de sangre. El pronóstico es diferente de unos casos a otros, según su localización. En total fallecieron 9 pacientes; seis de siete con hemorragias intracerebrales, dos de 14 con hemorragias subdurales y uno de cinco con una hemorragia en el conducto raquídeo.
