Zusammenfassung

Bei 140 Patienten im Alter zwischen drei Wochen und 83 Jahren wurden 143 perkutane transhepatische Cholangiographien vorgenommen. In 97 von 128 Fällen (76%) war die Punktion erfolgreich, in 95 Fällen (75%) konnte die Diagnose durch Operation, Leberpunktion oder Autopsie bestätigt werden. Aus diesen Erfahrungen geht hervor, daß beim ikterischen Patienten mit der perkutanen transhepatischen Cholangiographie schneller eine Diagnose und genauer eine präoperative Indikation zum chirurgischen Eingriff gestellt werden kann. Die Methode ist einfach, relativ komplikationsarm und empfehlenswert beim Verschlußikterus, besonders beim Ikterus nach Cholezystektomie, bei Verdacht auf Hepaticus- oder Choledochus-Steine sowie Tumoren der Gallengänge und des Pankreas.

Summary

143 percutaneous transhepatic cholangiographies were performed in 140 patients aged between three weeks and 83 years. In 97 (76%) percutaneous puncture was successful: in 95 the diagnosis was confirmed by operation, liver biopsy or necropsy. These results indicate that percutaneous transhepatic cholangiography provides quick diagnosis in jaundiced patients, giving also an exact indication for operative intervention. It is a simple method, relatively free of complications and is recommended if there is obstructive jaundice (particularly after cholecystectomy), suspected gall-stones, hepatic or common bile-duct stones or tumours of the biliary tract and pancreas.

Colangiografía percutánea transhepática

En 140 pacientes de edades comprendidas entre las tres semanas y los 83 años se llevaron a cabo 143 colangiografías percutáneas trans-hepáticas. En 97 casos (76%) fue la punción exitosa, en 95 casos (75%) pudo confirmarse el diagnóstico por la operación, punción hepática o autopsia. De estas experiencias se deduce que se puede establecer el diagnóstico más rápidamente por la colangiografía percutánea transhepática en el paciente ictérico, y establecer, con más exactitud, una indicación preoperatoria para la intervención quirúrgica. El método es simple, relativamente carente de complicaciones y recomendable en la ictericia obstructiva, sobre todo en la icteria consecutiva a la colecistectomía, ante la sospecha de cálculos hepáticos o del colédoco, así como en tumores de las vías biliares y del páncreas.
