Zusammenfassung

Die zentrale Hüftgelenksluxation stellt als Gelenksbruch sowohl therapeutisch als auch prognostisch noch immer große Probleme. In 20 Fällen wurde neben einem Längszug mit Kirschner-Draht-Extension durch die Femurkondylen ein seitlicher Schraubenzug in Richtung der Schenkelhalsachse angewandt. Bei fünf Patienten kam es dabei zu einem guten Spätergebnis (nach fünf Jahren), bei den übrigen zu einem guten bis zufriedenstellenden Früh- bzw. Mittelergebnis. In keinem Fall kam es zu einer Infektion durch die Schraube.

Summary

So-called central hip-joint dislocation (fracture of the acetabulum with profusion of the femoral head into the pelvis) remains a great problem both therapeutically and prognostically. In 20 cases it was treated by longitudinal traction using a Kirschner-wire extension through the femoral condyles and lateral pull in the direction of the axis of the femoral neck. Good results were obtained in five patients with a minimal follow-up of 5 years. In the others, results so far have also been good (follow-up less than 5 years). In no case was there infection at the site of the screw.

La llamada luxación central de la articulación coxofemoral y su tratamiento

La luxación central de la articulación coxofemoral plantea todavía grandes problemas como fractura articular, tanto terapéuticamente como pronósticamente. En 20 casos se empleó una tracción lateral en tornillo en sentido del eje del cuello femoral junto a una tracción longitudinal por extensión con alambre de Kirschner a través del cóndilo femoral. En 5 pacientes dio buenos resultados tardíos (después de cinco años), en el resto unos resultados tempranos a medio plazo buenos hasta satisfactorios. En ningún caso sobrevino una infección por el tornillo.
