Successful Treatment of Fulminant Hepatitis B during Pregnancy

Wir berichten über eine 28-jährige Frau in der 24. Schwangerschaftswoche, die sich mit einem fulminanten Leberversagen bei akuter Hepatitis-B-Infektion vorstellte. Es wurde eine Therapie mit Lamivudin (100 mg/tgl.) eingeleitet. Innerhalb der ersten Wochen nach Beginn der antiviralen Therapie sank die ALT bis zur Normalisierung rasch ab (Peak 5046 U/l). Nach knapp 10 Wochen war die Leberfunktion vollständig rekompensiert. Zudem trat bereits nach 3 Wochen eine HBe-Serokonversion ein, gefolgt von einer HBs-Serokonversion innerhalb von 6 Monaten. Fünf Wochen nach Beginn der Therapie mit Lamivudin (29. SSW) brachte die Frau auf natürlichem Geburtsweg ein frühgeborenes Kind zur Welt (Körpergewicht: 1000 g). Das Neugeborene erhielt innerhalb der ersten 12 Std. post partum eine simultane aktive und passive HBV-Immunisierung, bevor es aufgrund eines Mekoniumileus operiert werden musste. Im Rahmen der letzten Vorstellung, 241 Tage nach Beginn der Therapie mit Lamivudin, zeigten sowohl die Mutter als auch das Kind einen suffizienten anti-HBs-Titer, woraufhin die Therapie mit Lamivudin beendet wurde. Dieser Fall zeigt, dass eine Therapie mit Lamivudin in der Schwangerschaft bei fulminanter Hepatitis-B-Infektion möglich ist und helfen kann, sowohl eine Lebertransplantation als auch eine Fehlgeburt zu vermeiden.

Abstract

We here report the use of lamivudine 100 mg daily in a young pregnant woman (24th week of gestation) with fulminant hepatic failure due to acute HBV infection. After initiation of oral lamivudine (100 mg/d), ALT levels rapidly decreased from 5046 U/L to normal values within five weeks. HBe seroconversion occured three weeks after treatment start, followed by HBs seroconversion within less than six months. A preterm female baby was delivered at gestational week 29 (weight 1000 gr) (five weeks after start of lamivudine). The infant received simultaneous active and passive HBV immunisation within 12 hours after delivery. The neonatal check-up revealed meconium ileus which was successfully treated by surgery. At last presentation 241 days after initiation of treatment, both mother and infant showed stable HBs-seroconversion (anti-HBs 169 IU/mL and > 1000 IU/L, respectively). Therefore, lamivudine therapy was withdrawn. This case suggests that oral nucleos(t)ides may be safely used in pregnant patients with fulminant hepatitis B potentially preventing liver transplantation and interruption of pregnancy.
