Zusammenfassung

Die epithelialen Erhebungen und Polypen des Magens sind wegen ihrer Entartungsneigung und ihrer häufigen Koinzidenz mit Karzinomen von besonderem Interesse. Unter 28 Patienten fanden wir ein Adenom mit beginnender maligner Infiltration, einen hyperplastischen Polypen mit Zellatypien und fünf Fälle, bei denen gleichzeitig ein Karzinom und einzelne oder zahlreiche Polypen bestanden. Voraussetzung für eine planvolle chirurgische Therapie ist die exakte Diagnostik; hier hat die verbesserte Röntgentechnik in Kombination mit der Endoskopie und Biopsie gute Fortschritte gebracht. Sie ermöglichten bessere morphologische Kenntnisse dieser Erkrankung und eine brauchbare Klassifizierung der Polypentypen. Eine chirurgische Intervention wird nicht nur wegen der prospektiven oder bereits bestehenden Entartung notwendig, sondern auch deshalb, weil nicht selten andere gutartige Begleiterkrankungen dazu Anlaß geben.

Summary

Epithelial swellings and polyps of the stomach are of special interest because of their tendency towards malignant degeneration and the frequent coincidence of carcinoma. Among a group of 28 patients with benign neoplasm of the gastric mucosal epithelium (in some instances coincidental with gastric carcinoma) there was one case of adenoma with beginning malignant infiltration, one of hyperplastic polyps and five cases in which there was both carcinoma and one or numerous polyps. Exact diagnosis, by radiological means combined with endoscopy and biopsy, is essential for planned surgical treatment. Recent progress in diagnostic techniques has provided a better knowledge of the morphology of these lesions and has led to a practical classification of types of polyps. Surgical intervention becomes necessary not only because of any already existing or likely malignant degeneration, but also not uncommonly for accompanying benign diseases.

Resumen

Pólipos gástricos Morfología, clínica y terapéutica

Las elevaciones epiteliales y los pólipos del estómago tienen un especial interés por su propensión a la degeneración y su frecuente coincidencia con los carcinomas. Entre 28 pacientes encontramos nosotros un adenoma con infiltración maligna incipiente, un pólipo hiperplásico con atipias celulares y cinco casos en los que existían al mismo tiempo un carcinoma y pólipos numerosos o aislados. Condición previa para la indicación terapéutica quirúrgica es el diagnóstico exacto, habiendo aportado buenos resultados aquí las técnicas radiológicas perfeccionadas en combinación con la endoscopia y la biopsia. Estas facilitan mejores conocimientos morfológicos de esta afección y una clasificación utilizable de los tipos de pólipos. La intervención quirúrgica será necesario no solo debido a la degeneración prospectiva o ya existente, sino también porque así lo requieren no raramente otras afecciones concomitantes benignas.
