Zusammenfassung
Von 1966 bis 1971 wurden 262 Noteingriffe am Gefäßsystem wegen akuter Ischämie vorgenommen. Arterielle Embolien waren daran mit 60,7% (n = 159), arterielle Thrombosen mit 33,1% (n = 87) beteiligt. Bei arterieller Embolie gelang es, durch indirekte Embolektomie in 82,4% der Fälle (n = 131) die betroffene Extremität ohne funktionelle Beeinträchtigung zu erhalten, in 4,4% (n = 7) war der Gliedmaßenverlust nicht zu verhindern; die Gesamtletalität infolge der Grundkrankheit betrug 23% (n = 37). Bei arterieller Thrombose konnte nur in 59 der 87 Fälle die betroffene Extremität durch Embolektomie ohne funktionelle Einbuße erhalten werden, in 17 Fällen mißlang der Versuch; zwölf Patienten starben infolge der Grundkrankheit. Bei der Fernembolektomie wird die Anwendung des Fogarty-Katheters, bei der arteriellen Thrombose die Kombination von Ring-Stripper und Ballonsonde als Methode der Wahl angesehen.
Summary
From 1966 to 1971 a total of 262 emergency operations were performed because of acute ischaemia at the surgical clinic of Giessen University. Of these 159 (60.7%) were for embolism, eighty-seven (33.1%) for thrombosis. Indirect embolectomy succeeded in preserving the affected limb without functional impairment in 131 cases (82.4%) of arterial embolism, while in seven (4.4%) amputation could not be prevented. The overall death-rate from the underlying disease was 23% (37 patients). But in only 59 of the 87 cases of arterial thrombosis was it possible to preserve the limb without significant functional impairment after embolectomy, the operation failed in 17, and 12 patients died of the underlying disease. In distal embolectomy the Fogarty balloon catheter is the method of choice, while the combined use of ring stripper and Fogarty catheter is best in the treatment of arterial thrombosis.

El síndrome isquémico agudo Intervención de urgenica en la embolia y trombosis arteriales
Desde 1966 a 1971 se llevaron a cabo 262 intervenciones de urgencia en el sistema vascular por isquemia aguda. Las embolias arteriales participaban con un 60,7% (n = 159) y las trombosis arteriales con un 33,1% (n = 87). En las embolias arteriales se logró conservar por embolectomía indirecta la extremidad afectada sin merma de su función en el 82,4% de los casos (n = 131), no pudiéndose evitar la pérdida del miembro en el 4,4% de los casos (n = 7); la letalidad global consecutiva a la enfermedad principal alcanzó el 23% (n = 37). En sólo 59 de los 87 casos de trombosis arterial pudo conservarse por embolectomía la extremidad afectada sin pérdida funcional, fracasando el intento en 17 casos; 12 pacientes fallecieron a consecuencia de la enfermedad principal. Para la embolectomía a distancia se considera como método de elección el empleo del catéter de Fogarty, y en las trombosis arteriales la cominación de la cuerda anular y sondas de balón.
