Häufigkeit diabetischer Stoffwechselstörungen bei Herzinfarktpatienten

Zusammenfassung

80 Infarkt-Patienten, bei denen der Infarkt länger als zwölf Monate zurücklag, wurden mit dem Tolbutamid- und intravenösen Glucosetoleranz-Test auf diabetische Stoffwechselstörungen untersucht. Patienten mit bereits bekanntem Diabetes wurden nicht in die Untersuchung einbezogen. Bei der Auswertung des Tolbutamidtests nach Unger und Madison ergab sich für rund 40% der Untersuchten eine diabetische Stoffwechsellage. Unter Heranziehung der von Lundbaek angegebenen Daten für die Glucose-Assimilationskonstante k fanden wir rund 27% pathologische Werte. Offensichtlich ist aber der k-Wert von Lundbaek zu niedrig angegeben. Bei Anwendung eines rechnerisch ermittelten k von 1,4 als unterstem Normwert korrelieren beide Tests im Gruppen- wie im Einzelvergleich eng. Aus den Testergebnissen und unter Berücksichtigung der bereits bekannten Fälle von Diabetes ergibt sich eine Glucosestoffwechselstörung bei rund 50% aller Patienten mit Herzinfarkt.

Summary

80 patients who had sustained myocardial infarction at least 12 months previously were tested for possible diabetic metabolic disorders by the tolbutamide and intravenous glucose tolerance tests. Known diabetics were excluded. Analysis of the results of the tolbutamide test according to the criteria of Unger and Madison indicated that in about 40% of those studied there was evidence of a diabetic metabolic state. Using the data published by undbaek for the glucose assimilation constant »k« abnormal values were obtained in about 27% of cases. However, the k value of undbaek is apparently too low. Using a calculated k of 1.4 as the lowest normal level there was good correlation between the two tests, both for the groups as a whole and individual patients. These results and those previously reported indicate that a disorder of glucose metabolism can be expected in about half the patients with previous myocardial infarction.

Resumen

Frecuencia de los trastornos metabólicos diabéticos en los pacientes con infarto cardíaco Exploraciones paralelas con dos pruebas

En 80 pacientes con infarto cardíaco ocurrido hacía ya más de doce meses se exploraron los trastornos del metabolismo diabético con la prueba de la tolbutamida y de la tolerancia a la glucosa intravenosa. Los pacientes cuya diabetes era ya conocida no fueron incluídos en dichas exploraciones. Al valorar la prueba a la tolbutamida según Unger y Madison se encontró en el 40% de los explorados una situación metabólica diabética. Incluyendo los datos dados por Lundbaek para la constante k de asimilación de glucosa encontramos cerca de un 27% de valores patológicos. Sin embargo, el valor de la constante k de Lundbaek parece haberse interpretado como muy bajo. Empleando un valor k de 1,4, emitido por cálculo, como valor normal más bajo, se correlacionan estrechamente ambas pruebas en la comparación individual y por grupos. De los resultados de las pruebas y teniendo en cuenta los casos de diabetes ya conocidos resulta que casi en el 50% de todos los pacientes con infarto cardíaco existe un trastorno del metabolismo de la glucosa.
