Zusammenfassung

Innerhalb nur eines Vierteljahres wurden in der Klinik drei Kinder beobachtet, bei denen sich im Verlauf einer Behandlung mit Erythromycin-estolat (Neo-Erycinum®) ein Arzneimittelikterus ausbildete. Es handelt sich dabei überwiegend um eine intrahepatische Cholestase, die nur durch die Verbindung Propionyl-erythromycin-laurylsulfat, nicht aber durch andere Erythromycin-Abkömmlinge ausgelöst wird. Das Initialstadium ist sowohl bei Erwachsenen als auch bei Kindern gekennzeichnet durch Nausea, Appetitmangel, Erbrechen, Fieber und Brust- oder Oberbauchschmerzen. Ihnen folgt unmittelbar die Gelbsucht, die mit mehr oder weniger deutlich ausgeprägten klinisch-chemischen Veränderungen einhergeht und nach Fortlassen des Medikaments ebenso wie die anderen Symptome relativ rasch abklingt. Bei Reexposition mit derselben Erythromycin-Verbindung treten die gleichen Krankheitserscheinungen in kürzerem Abstand wieder auf. Es empfiehlt sich, bei der antibiotischen Therapie im Falle einer Erythromycin-Indikation im Kindesalter auf das Estolat zu verzichten und statt dessen andere Erythromycin-Präparate anzuwenden. Zudem sollte bei jedem nicht geklärten Ikterus stets auch an die Möglichkeit einer Arzneimittelschädigung gedacht werden.

Summary

Within a three-month period three children were observed in the Children's Clinic of the Municipal Hospital, Krefeld, because of jaundice which had developed in the course of treatment with erythromycin estolate (Neo-Erycinum®). The predominant cause was intrahepatic biliary stasis, precipitated by propionyl-erythromycin-lauryl sulphate, but not by other erythromycin derivatives. The initial stage is characterized both in adults and in children by nausea, lack of appetite, vomiting, fever and chest or upper abdominal pain. This is followed immediately by jaundice, which is associated with more or less marked clinical and biochemical changes, and rapidly disappears, as do the other signs and symptoms, immediately after the drug has been stopped. When the same erythromycin preparation is again administered, the same signs and symptoms quickly re-appear. It is recommended not to use the estolate but other erythromycin preparations when antibiotic treatment with erythromycin is indicated in children. Furthermore, in every case of jaundice of unknown cause the possibility of a drug effect should be considered.

Ictericia tras el tratamiento con estolato de eritromicina

En el plazo de sólo un trimestre fueron observados en la clinica tres niños, en los que apareció una ictericia medicamentosa en el transcurso de un tratamiento con estolato de eritromicina (Neo-Erycinum®). Predominaba una colestasis intrahepática desencadenada sólo por el preparado de laurilsulfato de propionileritromicina y no por otros derivados de la eritromicina. El estadio inicial se caracteriza tanto en adultos como en niños por náuseas, inapetencia, vómitos, fiebre y dolores epigástricos y torácicos. A continuación le siguen la ictericia acompañada de alteraciones clínico-químicas, marcadas con más o menos notoriedad, y que declinan con relativa rapidez, al igual que los otros síntomas, una vez suprimido el medicamento. Reexponiéndoles al mismo compuesto de eritromicina, aparecen de nuevo las mismas manifestaciones patológicas al poco tiempo. Se recomienda renunciar al estolato en el tratamiento antibiótico de eritromicina en la infancia y emplear, en su lugar, otro preparado de eritromicina. Además se debería pensar siempre en la posibilidad de una lesión medicamentosa en toda ictericia no aclarada.
