Zur Kombinationsbehandlung der akuten Leukämie des Kindes mit Dauno-Rubidomycin

Zusammenfassung

Bei 16 Behandlungen maligner Erkrankungen kam es zu stärkerer Leukozytendepression und einer hohen Zahl an septischen Komplikationen, wenn Daunoblastin in einer Dosis von 1,2 mg/kg gegeben wurde. Bei einer Dosis von 1,0 mg/kg war dies weit weniger der Fall. Das Medikament wurde nur einmal pro Woche gegeben. Es wurde mit Vincristin und Prednison kombiniert, in vier Fällen zusätzlich mit L-Asparaginase. Drei solide Tumoren sprachen nicht an. Ein sechsjähriges Mädchen mit akuter Leukämie verstarb zu Behandlungsbeginn, in allen anderen Fällen wurden klinisch und hämatologisch Vollremissionen erzielt. Die therapeutische Breite des Daunoblastin ist gering. Es ist anzunehmen, daß bei Kombinationstherapie die Dosis von 1,0 mg pro Kilogramm Körpergewicht und pro Woche in vielen Fällen in den noch therapeutisch wirksamen, aber noch nicht in den höher toxischen Bereich fällt.

Summary

Twelve children with acute lymphoblastic leukaemia were treated with daunoblastin, combined with prednisone (2.5 mg/kg daily) and vincristin (0.05 mg/kg daily), four of them also receiving L-asparaginase (200 U/kg daily for 28 days). Daunoblastin at a dosage of 1.0 mg/kg once weekly i. v. was well tolerated (10 children), but in six children, who received 1.2 mg/kg there was marked depression of leucocytes with a high incidence of septic complications. There was no therapeutic response in three additional children with solid malignant tumours. Except in one case (a six-year-old girl who died of acute leukaemia at the onset of treatment) full clinical and haematological remission was achieved in all cases of leukaemia. The therapeutic range of daunoblastin proved to be very narrow. However, when administering it, as in this series, at a dose of 1.0 mg/kg body-weight/week, it is likely that it will be therapeutically effective in many cases without causing severe toxic reactions.

Acerca del tratamiento combinado de la leucemia aguda del niño con dauno-rubidomicina

En 16 tratamientos de afecciones malignas se presentó una depresión leucocitaria más intensa y un mayor número de complicaciones sépticas cuando se administró daunoblastina en dosis de 1,2 mgr/kgr, siendo menos frecuentes en dosis de 1,0 mgr/kgr. El medicamento se administró tres veces por semana. Se le combinó con vincristina y prednisona y en cuatro casos además con L-asparginasa. Tres tumores sólidos no respondieron. Una niña de seis años con leucemia aguda falleció al comienzo del tratamiento, lográndose en todos los restantes casos totales remisiones clínicas y hematológicas. El espectro terapéutico de la daunoblastina es pequeño. Es de suponer que en la terapéutica de combinación cae en muchos casos la dosis de 1,0 mgr/kgr de peso corporal y por semana bajo los márgenes terapéuticos activos y no de grave toxicidad.
