Zusammenfassung
Bei einem 38jährigen Patienten mit einer Achalasie kam es in Abhängigkeit von der Nahrungsaufnahme zu synkopalen Anfällen, die sich im Elektrokardiogramm als totaler atrioventrikulärer Block manifestierten. Diese elektrokardiographischen Veränderungen waren durch Ballondehnung des terminalen Ösophagus jederzeit reproduzierbar. Nach Implantation eines QRS-gesteuerten Schrittmachers konnte die mechanische Dilatation des ösophago-kardialen Überganges ohne Komplikationen vorgenommen werden. Seitdem sind keine synkopalen Anfälle, für die ein ösophago-kardialer Reflex bei einem hypersensitiven Ösophagus angenommen wurde, mehr aufgetreten.
Summary
In a 38-year-old patient with achalasia syncopal attacks occurred on swallowing food. They proved to be due to total A-V block and the electrocardiographic evidence could be reproduced at will by dilating the terminal oesophagus with balloon. After implantation of a QRS-triggered pacemaker cardiac standstill no longer occurred on mechanical dilatation of the oesophago-cardiac junction and syncopal attacks ceased. It is thought that they were due to an oesophago-cardiac reflex from a hypersensitive oesophagus.

Paro cardíaco asistólico en la acalasia
En un paciente de 38 años con acalasia se presentaron ataques sincopales dependientes de la ingestión de alimentos, que se manifestaron en el electrocardiograma como bloqueo atrioventricular completo. Estas alteraciones electrocardiográficas eran reproducibles en todo momento por la dilatación globulosa del esófago terminal. Después de la implantación de un marcapaso dirigido por el complejo QRS, pudo llevarse a cabo sin complicaciones la dilatación mecánica del tránsito esófago-cardias. Desde entonces no han aparecido más ataques sincopales para los que pudiese admitirse un reflejo cardioesofágico en un esófago hipersensible.
