Zusammenfassung

Es wird das methodische Vorgehen bei der Anwendung zytochemischer Phosphatasetests sowohl nach dem Calcium-Cobalt-Verfahren (Gömöri) als auch nach dem Azokupplungs-Verfahren (Kaplow) in den von der Freiburger Klinik benutzten Modifikationen dargelegt. Die Prinzipien quantitativer Auswertung behandelter Blutausstriche werden erörtert und die Bedeutung der Aktivitätszahl geschildert. Es zeigt sich, daß Blutausstriche hinsichtlich ihrer Phosphatase-Aktivität mittels einer einzigen Zahl gut zu charakterisieren sind. Bei den aplastischen Anämien läßt sich eine besondere Gruppe mit niedriger oder fehlender Phosphatase-Aktivität abgrenzen, die möglicherweise präleukämischen Zustandsbildern zuzuordnen ist. Einzelne Krankheitsfälle werden erwähnt. Statistische Berechnungen der Fehlerquote zeigen, daß auß bei Erkrankungen mit normaler oder mäßig erhöhter Fermentaktivität durch mehrfache Untersuchungen eine Abgrenzung gegenüber der chronischen myeloischen Leukämie erreicht werden kann.

Summary

Histochemical methods for the intra-cellular demonstration of alkaline phosphatase (calcium-cobalt test of Gömöri and the azo-coupling test of Kaplow) and their personal modifications are described. The principles underlying the quantitative estimation of treated blood smears and the significance of activity levels are discussed. It is shown that, as far as phosphatase activity is concerned, blood smears can be clearly defined by means of one value. Among the aplastic anaemias there is one group which is characterized by a low or zero phosphatase activity, which is possibly the results of a pre-leukaemic state. Some case-reports are presented. Calculation of the standard error shows that even in diseases with normal or moderately elevated enzyme activity several determinations allow the differentiation from chronic myeloid leukaemia.

La fosfatasa alcalina de leucocitos neutrófilos

Los autores exponen la metodología práctica de la aplicación de tests citoquimicos de fosfatasa tanto según el procedimiento de calcio-cobalto (Gömöri) como según el procedimiento de azo-acoplamiento (Kaplow) con las modificaciones utilizadas en la clínica de Friburgo. Describen los principios de la valoración cuantitativa de frotes sanguíneos tratados y la significación del número de actividad. Resulta que frotes sanguíneos pueden caracterizarse respecto de su actividad fosfatásica de un modo inequívoco mediante un solo número. En las anemias aplásticas es posible delimitar un grupo especial con actividad fosfatásica reducida o nula, que tal vez podrá ser clasificado entre los cuadros de estado preleucémico. Se mencionan algunos cuadros nosológicos. Cálculos estadísticos relativos a la cuota de error muestran que también en afecciones con actividad fermentativa normal o moderadamente aumentada es posible lograr una delimitación frente a la leucemia mieloide crónica, gracias a exploraciones múltiples.
