Die Verkennung des akuten Glaukomanfalls - eine verhängnisvolle Fehldiagnose

Zusammenfassung

Unter Hinweis auf die nicht seltene Verkennung des akuten Glaukomanfalls durch den praktischen Arzt oder durch den Krankenhausarzt wird die Symptomatik des akuten Glaukomanfalls kurz geschildert. Als auslösende Ursachen kommen die Pupillenerweiterung und die Erregung des vegetativen Nervensystems durch Allgemeinkrankheiten, seelische Belastung, Witterungseinflüsse und toxische Wirkungen in Betracht. Verhängnisvoll ist die Verkennung des akuten Glaukomanfalls deswegen, weil schnelle Behandlung notwendig ist, da schon wenige Stunden nach Beginn des Anfalls irreparable Sehstörungen entstehen können.

Summary

Five case histories are presented to demonstrate the damage to vision which may result when an attack of acute glaucoma is misdiagnosed and treatment delayed for more than a few hours. The characteristic signs and symptoms of such an attack are described: the most important are pain (ranging from dull ache to pain of unbearable severity) and visual symptoms, often described as “foggy vision”. Short prodromal symptoms are common; they consist of the illusion of coloured rings around burning light bulbs when viewed in the dark. The eyeball is often reddened during the attack (“congestive glaucoma”) and, with the increase in intra-ocular pressure, it feels very hard to palpation. — It is pointed out that a variety of factors may precipitate an acute attack, such as pupillary dilatation, stimulation of the autonomic nervous system by some other disease (influenza, pneumonia, stroke), emotional upset, weather changes, and toxic substances (alcohol, caffeine). Irreversible damage to vision will occur within a few hours of the onset of an attack, unless it is treated.

El desconocimiento del ataque de glaucoma agudo, un diagnóstico erróneo fatal

Se describe brevemente la sintomatología del ataque de glaucoma agudo, señalando el hecho de ser frecuentemente objeto de un diagnóstico erróneo por parte del médico práctico o médico de hospital. Como causas provocadoras entran en consideración la dilatación de la pupila y la excitación del sistema nervioso vegetativo por afecciones generales, cargas psíquicas, influencias meteóricas y efectos tóxicos. El desconocimiento del ataque de glaucoma agudo es fatal, porque éste requiere un tratamiento inmediato, pudiendo originarse trastornos visuales irreparables ya a pocas horas de empezar el ataque.
