Zusammenfassung

Es wird über Erfahrungen mit der Konvergenzbestrahlung berichtet, die bei 66 Kranken, 64 Männern und 2 Frauen, mit einem inoperablen Bronchialkarzinom in den letzten 4 Jahren gesammelt werden konnten. Mit dieser Methode kann man in 11 cm Tiefe eine Herddosis von 5000 r geben, während die Haut nur mit einer Dosis von 800—1000 r belastet wird. Praktisch ist es aber schwierig, den Strahlenkegel der Größe des Herdes anzupassen und auf den Herd einzustellen. Von den Bestrahlten leben jetzt noch 19 Kranke. Bei 3 Patienten liegt der Beginn der Röntgenbestrahlung erst etwa œ Jahr zurück. Eine eindeutige Lebensverlängerung gegenüber der Behandlung mit den bisher üblichen Methoden scheint die Konvergenzbestrahlung aber nicht zu bewirken, wie ein Literaturvergleich zeigt. Die Methode ist jedoch erheblich angenehmer, insbesondere für die Haut schonender als alle anderen Verfahren. Es wird eine Konvergenzbestrahlung vor der Operation des Bronchialkarzinoms vorgeschlagen, um möglicherweise die heute noch sehr ungünstigen Behandlungsergebnisse zu bessern. Da die Strahlendosis, die bei dieser Methode die ableitenden Lymphbahnen trifft, wesentlich geringer ist als die Herddosis, wird eine zusätzliche Stehfeld-Bestrahlung der Lymphabflußwege für zweckmäßig gehalten. Diese Vorschläge können noch nicht durch eigene Erfahrungen gestützt werden.

Summary

Results are reported in 66 cases (64 males, 2 females) with inoperable bronchial carcinoma which had been seen during the last 4 years and who were treated with convergence radiation. By this method a focal dose of 5,000 r can be delivered at a depth of 11 cm., while the cutaneous dose is between 800 and 1000 r only. In practice, however, it is often difficult to adjust the radiation cone to the size of the tumour and to focus it accurately. Out of the group- of 66 patients, 19 are still alive. But in 3 of these, radiation was performed only 6 months ago. A review of the literature shows that this method of treatment gives no unequivocal increase in survival time when compared with more usual methods of treatment. It is, however, a more comfortable method for the patient, because it affects the skin much less. The pre-operative use of this method is advocated. It is pointed out that the lymphatic drainage channels receive less dose than the tumour itself. Additional stationary-field radiation of lymphatic channels should, therefore, be undertaken. However, the author points out that his own results do not yet provide numerical evidence for the benefit of these recommendations.

La radiación convergente del carcinoma bronquial

Se informa sobre la experiencias reunidas en los últimos 4 años acerca de la radiación convergente efectuada a 66 enfermos, 64 hombres y 2 mujeres, afectados de un carcinoma bronquial inoperable. Con este método se puede dar una dosis focal de 5000 r a 11 cm de profundidad, mientras que la piel sólo sufre una dosis de 800—1000 r. Pero prácticamente es difícil adaptar el cono de rayos al tamaño del foco, así como acomodarlo a él. 19 de los enfermos irradiados viven aún. En 3 pacientes, data el comienzo de la radioterapia aprox. desde hace medio año. La radiación convergente no parece conseguir una clara prolongación de la vida al igual que con los métodos de tratamiento hasta ahora habituales, como demuestra una comparación bibliográfica. No obstante, el método es mucho más atrayente, especialmente por respetar mejor la piel que con todos los demás procedimientos. Se propone una radiación convergente antes de operar el carcinoma bronquial, para mejorar en lo posible los hasta hoy aún muy desfavorables resultados del tratamiento. Ya que la dosis de radiación que con este método alcanza las vías linfáticas derivadas es bastante más inferior que la dosis focal, se recomienda como útil una radiación en campo fijo adicional de las vías linfáticas de desagüe. Estas proposiciones aún no han podido ser confirmadas por experiencias propias.
