Über die Phimose

Zusammenfassung

Der Begriff der Phimose wird erläutert.

Ihre häufigste Form, die Verklebungsphimose, die in Wirklichkeit keine Vorhautverengung darstellt, wird besonders herausgestellt und das Verfahren zur Behebung dieser Veranderung geschildert.

Diese Verklebung kann nur durch eine Losung beseitigt werden.

Das immer noch zu beobachtende folgenschwere Dehnen des Praputialrohres muß endlich der Vergangenheit angehoren. Bei der Verklebungsphimose ist es sinnlos, schädlich und führt durch Narbenbildung zu echten Verengungsphimosen, und bei diesen kann nur die operative Korrektur beider Formen, der hypertrophischen und atrophischen, nützen.

Die echte Phimose richtig zu erkennen, die Verklebung aber sachgemäß zu lösen, ist Aufgabe des praktischen Arztes, der damit seinen kleinen Patienten manches ersparen kann.

The meaning of phimosis is discussed. Its most frequent form, i.e. that due to adhesion which, in fact, is not a constriction of the praepuce proper, is dealt with in detail, and the method for its treatment is described. The adhesion can only be removed by separation and loosening. The still practised dilatation of the praepuce should by now be a thing of the past because of the inherent risk of serious consequences. If it is a case of simple adhesion it is nonsensical, harmful and may lead to a phimosis proper, i.e. constriction of the praepuce, through scar formation. In the latter case only surgical intervention can remedy the hypertrophic or atrophic changes. To diagnose the latter correctly, and to separate the adhesion properly is the task of the general practitioner who will thereby save his small patient much pain and discomfort.

El autor aclara el concepto de fimosis insistiendo especialmente sobre su principal forma, la fimosis por adherencia, que no constituye en realidad una estrechez del prepucio, y describe el procedimiento para tratarla. Esta adherencia sólo puede resolverse de una manera. La dilatación del prepucio, que puede tener graves consecuencias, se practica todavía hoy demasiado; debe abandonarse definitivamente; en la fimosis por adherencia, no tiene sentido, es perjudicial y formando cicatrices puede conducir a una verdadera fimosis estenótica; en ésta, ya se trate de la forma atrófica o de la hipertrófica, sólo una corrección operatoria puede representar una utilidad. Es tarea del médico práctico distinguir la verdadera fimosis y tratar adecuadamente las adherencias, evitando de esta manera molestias a los pequeños pacientes.
