Zusammenfassung

Pyknoleptische Anfälle unterscheiden sich elektroencephalographisch von kleinen epileptischen Anfällen eines bestimmten Erkrankungsalters nicht. Pyknolepsien sind daher eine bestimmte Form von Epilepsie, deren Stellung im Rahmen anderer Epilepsieformen genauer zu bestimmen ist. Die Prognose ist nicht so günstig, wie angenommen wurde. Spontanheilungen sind kaum häufiger als bei anderen Epilepsieformen. Zum Teil persistieren die kleinen Anfälle, bei der Mehrzahl kommen große Anfälle hinzu. Die Erkrankung beginnt vom 4.—14. Lebensjahr, meist aber zwischen dem 6.—8. Jahr. Die Anfälle sind mild, dauern kurz und gehen mit Bewußtseinspausen einher. Bei feinerer Symptomanalyse ergeben sich verschiedene Anfallssyndrome: sensorisch und motorisch indifferente Absencen, Petit Mal mit oralen Symptomen oder mit propulsiven, adversiven oder richtungsindifferenten Bewegungen. Pyknoleptische Anfälle stellen daher Rudimentärformen jeder Art von Petit Mal dar. Am häufigsten und allein auf das Pyknolepsiealter begrenzt sind Anfälle mit retropulsiven Bewegungen der Augen, des Kopfes und des Rumpfes. Diese sogenannten Retropulsiv-Petit Mal bilden symptomatologisch, pathophysiologisch und biologisch zusammen mit Propulsiv-Petit Mal (Blitz-, Nick- und Salaamkrämpfen) und mit Impulsiv-Petit Mal (secousses) eine Petit Mal-Trias. Die später hinzutretenden großen Anfälle spielen sich nach Art sogenannter „Aufwachepilepsien” —nach dem Erwachen und in Entspannungssituationen ab. Die Verwandtschaft der Pyknolepsie zur Aufwachepilepsie zeigt sich ferner in der gemeinsamen Bindung der Anfälle an bestimmte Gelegenheiten, in ihrer starken Abhängigkeit von äußeren Momenten, einer besonderen somatopsychischen Konstitution und einer erhöhten spezifischen Heredität. Medikamentös wirken Hydantoine schlecht, Barbiturate gut und Oxazolidine am besten.

Summary

A catamnestic assessment of 163 cases of pyknolepsy shows clearly that the disorder belongs nosologically to epilepsy, and that the pyknoleptic attacks are a variant of petit mal fits. The changes in the E.E.G. confirm this view. The prognosis of the disorder is not as favourable as is generally assumed. Of the cases observed over long periods only 16 p.c. were cured; 10 to 14 years after the onset of the disorder 31 p.c. still had pyknoleptic attacks; in 53 p.c. grand mal seizures had developed. Pyknolepsy seems, therefore, to be the precursor of grand mal epilepsy. The disease starts between the age of 4—14 years, with a maximum incidence between the age of 6 to 8 years. An analysis of the symptoms reveals that pyknolepsy may manifest all the various types of epileptic fits, but in a very mild forme. Retro-pulsive movements are most frequently seen. Mental development is frequently retarded. Treatment with hydantoin compounds proved disappointing; barbiturates gave better results. Oxazolidines had the best effects, but their use is limited by their toxic effects on the blood.

La posición clínica de la picnolepsia

La valoración catamnéstica de 163 observaciones de picnolepsia mostró claramente que dicho transtorno corresponde nosológicamente a la epilepsia y que los accesos picnolépticos son una variedad de los accesos de pequeño mal. Los transtornos del EEG hablan igualmente a favor de esta interpretación. El pronóstico de este transtorno no es tan favorable como generalmente se supone. Después de larga observación, sólo evolucionaron hacia la curación el 16% de los casos. El 31% de los casos tenían todavía accesos picnolépticos 10 a 14 años después del comienzo de la enfermedad. Y el 53% restante presentaban después de este tiempo accesos de gran mal. Asi pues, la picnolepsia representa la antesala de la epilepsia. El comienzo de la enfermedad oscila entre los 4 y los 14 años, con un máximo a los 6—8 años. El análisis de los síntomas muestra que en la picnolepsia se pueden presentar todas las formas de ataque epiléptico y singularmente la forma benigna. Los movimientos retropulsivos son frecuentísimos. El desarrollo psíquico y anímico está alterado con frecuencia. Los preparados de hidantoína no poseen eficacia terapéutica. Se han mostrado más útiles los barbituratos. Las oxazolidinas actúan muy eficazmente sin embargo su empleo está limitado por los efectos tóxicos sobre el cuadro hemático.
