Die subtotale Pankreasresektion zur Behandlung eines nicht auffindbaren Inseladenoms

Zusammenfassung

Es wird über einen Kranken mit einem kopfnahen Inseladenom des Pankreas berichtet, bei dem eine völlige Symptomfreiheit durch eine subtotale Resektion des Pankreas erzielt wurde.

Eine kritische Auswertung der veröffentlichten Inseladenom-Operationen zeigt in Übereinstimmung mit unserem Befund, daß die postoperative Hyperglykämie beim Inseladenom um so geringer ist, je umfangreicher die Resektion war.

Summary

A report on a patient suffering from an adenoma of the islands near the head of the pancreas; a subtotal resection of the pancreas resulted in complete freedom from symptoms. A critical evaluation of the published results of operations for island adenoma showed, in agreement with our own results, that in these cases the postoperative hyperglycemia is in inverse proportion to the extent of the excision. Subtotal pancreatectomy is, therefore, chosen in preference to any other form of partial resection; or to enucleation of the adenoma.

La resección subtotal del páncreas como tratamiento de un adenoma insular que no se encuentra en la operación

Se refiere el casó de un enfermo con un adenoma insular del páncreas próximo a la cabeza que quedó absolutamente libre de síntomas después de una resección subtotal del páncreas. Una valoración crítica de las operaciones sobre el adenoma insular publicadas muestra, de acuerdo con nustro hallazgo, que la hiperglucemia post-operatoria que tiene lugar después de la extirpación del adenoma insular es tanto más pequeña cuanto más amplia fué la resección. Por consiguiente, se prefiere la resección subtotal del páncreas en el adenoma insular a todas las demás resecciones parciales así como a la enucleación del adenoma.
