Zusammenfassung

Von September 1952 bis Ende Dezember 1956 wurden an der Chirurgischen Klinik der Freien Universität Berlin 250 Lungenresektionen wegen einer Tuberkulose durchgeführt. Darunter fanden sich 51 teilweise eingeschmolzene, teilweise kavernisierte Tuberkulome. Es wird ein Überblick aus röntgenologischer, klinischer und pathologisch-anatomischer Sicht zum Thema Tuberkulom gegeben. An eigenen Erfahrungen und Angaben in der Literatur werden Klinik, Verlauf sowie die konservative und chirurgische Behandlung besprochen. Der ungünstige Einfluß der Chemotherapie auf das Tuberkulom, das besonders im inaktiven Stadium unter dem Einfluß der Tuberkulostatika zur Einschmelzung kommen kann, wird besprochen. Von 51 chirurgisch behandelten Fällen konnten 49 als geheilt betrachtet werden. Abschließend werden die für die Behandlung des Tuberkuloms wesentlichen Gesichtspunkte herausgestellt.

Summary

Between September, 1952 and the end of December, 1956 250 lung resections for tuberculosis were performed at the Department of Surgery of the Free University of Berlin. Among these there were 51 tuberculomas, some of them cavernous (14), some with softening (32). Their size averaged 2—3 cm. in diameter. More than ⅔ of the patients were between 20 and 35 years of age and men were more commonly affected (30). By far the most common signs were cough (33), positive sputum (27) and increased E.S.R. (28). Solitary tuberculomas were present in 24. Perifocal spread was noticed in an additonal 19 cases. The unfavourable effects of chemotherapy on tuberculoma is discussed: they tend to soften and break down, especially in the inactive stage. Of the 51 observed cases, segmental resection was performed in 41, wedge resection in 3, lobectomy in 6 and pneumonectomy in 1. In all cases the sputum became or remained negative postoperatively. 49 of the patients were cured by the removal of the tuberculoma. The indications for operation are listed:- All tuberculomas larger than 2 cm. in diameter; smaller foci, which show evidence of activity (positive sputum, signs of softening or increase in size); foci suspected of being tumours.

Acerca del tratamiento quirúrgico del tuberculoma pulmonar

Desde septiembre de 1952 a finales de diciembre de 1956 se llevaron a cabo en la Clínica Quirúrgica de la Universidad Libre de Berlín 250 resecciones pulmonares por tuberculosis. Entre estos casos se encontraban 51 tuberculomas, en parte caseificados y en parte cavernizados. El autor hace una revisión del tema del tuberculoma desde el punto de vista radiológico, clínico y anatomo-patológico. A base de la propia experiencia y de los datos de la literatura se describe la clínica y evolución así como el tratamiento conservador y el quirúrgico. Se insiste sobre la influencia desfavorable de la quimioterapia sobre el tuberculoma el cual, singularmente en el estadio inactivo, puede evolucionar hacia la caseificación bajo el influjo de los tuberculostáticos. De los 51 casos tratados quirúrgicamente pueden considerarse curados 49. Finalmente se exponen algunos esenciales puntos de vista para el tratamiento del tuberculoma.
