Zusammenfassung
Spondylitis und Spondylodiszitis weisen eine zunehmende Inzidenz auf. Sie sind bei frühzeitiger Diagnosestellung oft konservativ gut therapierbar. Wichtig ist in jedem Fall die Keim– und Resistenzbestimmung. Bei neurologischem Defizit, Abszessformation, Instabilität und Fehlstellung der Wirbelsäule ist die operative Therapie zwingend. Die Verfahrenswahl beinhaltet primär oder sekundär nach vorheriger dorsaler Stabilisierung immer auch ein ventrales Vorgehen. Mit den modernen Methoden der Wirbelsäulenchirurgie kann dies sicher und mit gutem Outcome erfolgen.
Abstract
The incidences of spondylitis and spondylodiscitis are increasing steadily. When the diagnosis is made early the diseases are often amenable to conservative therapy. Determinations of pathogens and resistance are highly important in all cases. In cases with a neurological deficit, the formation of abscesses, instability and malposition of the spinal column operative therapy is mandatory. The available primary and secondary procedures include, after a prior dorsal stabilisation, also a ventral intervention. Good outcomes can be achieved safely with the methods on modern spinal surgery.
