Zusammenfassung
Handosteoarthrosen (HOA) stellen eine sehr häufige Erkrankung dar, welche zu einer
beträchtlichen Behinderung führen können. In ihrer Beachtung wurden sie lange Zeit
deutlich hinter Arthrosen der großen Gelenke wie der Hüfte oder dem Kniegelenk gereiht.
Das Assessment von Handosteoarthrosen mit der Anwendung von Krankheits-spezifischen
Scores ist mittlerweile von den Fachgesellschaften international gefordert und befürwortet
worden und beginnt sich zu etablieren, sowohl in klinischen Studien als auch als Verlaufsinstrument
in der täglichen Routine. Nach einer Zeit des Einsatzes unspezifischer Instrumente
wurden im Wesentlichen zwei Patienten-zentrierte Fragebögen entwickelt und in diesem
Patientenkollektiv validiert: der AUSCAN und der SACRAH mit seiner Weiterentwicklung
zum M-SACRAH und SF-SACRAH. Eine rezente Vergleichsarbeit attestierte sowohl dem AUSCAN
als auch dem M-SACRAH gleichwertige Einsetzbarkeit zum Assessment dieser Patienten.
Abstract
Hand osteoarthritis (HOA) is a highly prevalent condition, which can result in considerable
disability. In comparison with the diagnostic and therapeutic efforts concerning knee
or hip OA, HOA has been neglected for quite some time. The international rheumatological
community has agreed upon the necessity for the assessment of HOA by applying disease-specific
scores both for the performance of clinical trials and for use in daily clinical routine.
While unspecific scores have been used in former times, in the last few years two
specific scoring instruments have been created and validated in affected patients:
the AUSCAN and the SACRAH with its modifications into the M-SACRAH and the SF-SACRAH.
A recent publication dealt with the comparison of AUSCAN and M-SACRAH and revealed
the applicability and reliability of both instruments in this patient cohort.
