Chronic thrombotic scarring in patients with acute deep venous thrombosis of the lower limbs

Problemstellung:

Some patients with acute deep venous thrombosis of the lower limbs (DVT) may present risk factors (RF) for recurrent disease. We wanted to analyze some conditions related to reccurent DVT, other than the thrombophilias.

Patienten und Methode:

88 consecutive patients (47 males-53.41%, main age 64.9±13.9 years) were admitted in a Medical Clinic in 2007. Duplex ultrasonography assessed the acute DVT and the post-thrombotic scar (PTS). Clinical examination detected the principal RF. The 28 subjects with DVT and PTS formed the group A (31.82%). The 60 patients with DVT without SVT were included in the group B (68.18%).

Ergebnisse:

Only 17 subjects of the group A and 7 of the group B had personal history of DVT or pulmonary embolism (60.71% versus 11.67%, OR 11.7, p<0.0001). 9 subjects of the group A and 25 of the group B were 60–74 years (p=0.89). 9 patients of the group A and 13 of the group B were over 75 years (p=0.75). Other important RF in groups A and B were: varicose veins (14 vs. 24 patients-p=0.51), obesity (13 vs. 18, p=0.21), malignancy (6 vs. 8, p=0.25), chronic obstructive lung disease (5 vs. 6, p=0.24), bed rest (1 vs. 7, p=0.21), sepsis (1 vs. 4, p=0.49), major surgery (1 vs. 1, p=0.54), stroke (0 vs. 3, p=0.62), family history of DVT, immobilizing plaster cast, and congestive heart failure (0 vs. 1, p=0.54).

Schlussfolgerungen:

Almost 40% of patients with DVT and PTS had no personal history of venous thromboembolism. There were no significant RF in patients with DVT and PTS.
