Die Behandlung schwer- und schwerstverletzter Patienten stellt eine große Herausforderung für alle an der Versorgung beteiligten Fachdisziplinen dar. Die klinische und radiologische (Basis-)Diagnostik sollte dabei prioritätenorientiert erfolgen und sich dementsprechend an der Gesamtverletzungsschwere des Patienten orientieren. Für das operative Gesamtbehandlungskonzept bedeutet dies, dass es sich nach den Prinzipien des Damage Control richten sollte. Patienten im hämorrhagischen Schock sollten durch operative Eingriffe optimalerweise nicht zusätzlich kompromittiert werden. Ausgedehnte thorakale, abdominale und/oder unfallchirurgische Operationen sind daher auf ein Minimum zu reduzieren und das Hauptaugenmerk sollte in dieser Phase auf das Überleben des Patienten gerichtet sein.
