Zusammenfassung

Die Ultraschallkriterien zur Graduierung von Karotisstenosen bezogen sich in Deutschland bisher auf die lokale Durchmesserreduktion, entsprechend der in der europäischen ECST-Studie benutzten Definition. Um die Konfusion durch verschiedene nebeneinander benutzte Stenosegrad-Definitionen zu beseitigen, hat die interdisziplinäre S 3-Leitlinienkonferenz „Karotisstenose” nun die einheitliche Verwendung der in der amerikanischen NASCET-Studie verwendeten Graduierungssystematik (distaler Stenosegrad) empfohlen. Die erstmals 1986 publizierten Kriterien der Deutschen Gesellschaft für Ultraschall in der Medizin (DEGUM) wurden deshalb in NASCET-Stenosegrade transferiert. In Ergänzung der bisherigen Kriterien wird eine Einteilung in Haupt- und Zusatzkriterien vorgeschlagen und es werden neue Kriterien der farbkodierten Duplexsonografie eingefügt. Bei kombinierter Anwendung dieser Kriterien erlaubt der Ultraschall eine zuverlässige Stenosegraduierung der A. carotis interna. Dies gilt auch für eine Graduierung entsprechend der NASCET-Definition.

Abstract

Since 1986, German ultrasound criteria for grading carotid stenosis have followed the local diameter reduction percentage consistent with the definition used in the European Carotid Surgery Trial (ECST) definition. To overcome the confusion caused by the coexisting grading method used in the North American Symptomatic Carotid Endarterectomy Trial (NASCET), a German interdisciplinary council on carotid artery stenosis has recommended the implementation of the NASCET grading system (distal diameter reduction percentage) as the standard. The multi-parametric German ”DEGUM ultrasound criteria” consisting of combined Doppler and imaging criteria have consequently been revised and transferred to the NASCET definition. In addition, a novel differentiation between main (primary) and additional (secondary) criteria has been proposed. When these ultrasound criteria are combined, vascular sonography allows reliable grading of carotid disease.
