Gleichzeitige Anwendung von Thrombozytenaggregationshemmern und Protonenpumpeninhibitoren (PPIs)

Empfehlungen aktueller Leitlinien und Aussagen der amerikanischen (FDA) und europäischen (EMEA) Behörden

Die Deutsche Gesellschaft für Verdauungs- und Stoffwechselkrankheiten (DGVS) erstellte 2008 in Zusammenarbeit mit anderen wissenschaftlichen Fachgesellschaften die S 3-Leitlinie „Helicobacter pylori und gastroduodenale Ulkuskrankheit” [1]. Darin wird die Empfehlung ausgesprochen, „im Falle einer gleichzeitigen Thrombozytenaggregationshemmung mit ASS und Clopidogrel eine begleitende PPI-Medikation vorzunehmen” (Empfehlungsgrad C, Evidenzstärke 2a, Konsens). In dem nachfolgenden Kommentar wird ausgeführt, dass angesichts des unter der dualen Plättchenhemmung erhöhten Risikos für eine gastroduodenale Blutung die gleichzeitige Gabe eines PPI zu empfehlen sei, auch wenn es dafür keine direkt anwendbare Studie gibt.

Ebenfalls 2008 hatten sich mehrere amerikanische Fachgesellschaften (American College of Cardiology Foundation ACCF, American College of Gastroenterology ACG und American Heart Association AHA) zu einer Task Force zusammengefunden und einen Konsensusreport zu den gastrointestinalen Risiken der Thrombozytenaggregationshemmung und der Einnahme von nicht steroidalen Antirheumatika (NSAID) verfasst [2]. In diesem Konsensuspapier wird im Text die duale Plättchenhemmung mit ASS und Clopidogrel nicht direkt angesprochen. Es wird darauf hingewiesen, dass die Kombination von ASS bzw. Clopidogrel mit Antikoagulantien ein besonderes Risiko für schwere, insbesondere gastrointestinale Blutungen darstellt. Kardiovaskulärer Nutzen und gastrointestinale Risiken müssten daher individuell abgeschätzt werden. Bei simultaner Einnahme von ASS und Antikoagulantien sollten die Patienten eine begleitende PPI-Medikation erhalten. PPIs seien zudem die zu bevorzugenden Medikamente zur Prävention und Therapie ASS-induzierter gastrointestinaler Läsionen. Bei genauer Durchsicht des Reports findet sich darin ein Algorithmus, der bei der dualen Plättchenhemmung dann doch eine (im Text nicht ausgeführte) PPI-Komedikation empfiehlt. In einem im Dezember 2009 publizierten Update enthält sich das Expertenkommitee einer offiziellen Empfehlung, solange keine als Originalarbeit publizierten Ergebnisse aus randomisierten kontrollierten Studien vorliegen [3].

Die deutsche S 3-Leitlinie wie der amerikanische Konsensusreport nehmen auch Stellung zur Helicobacter-pylori-Eradikation in diesem Kontext. Bei Auftreten einer oberen gastroduodenalen Blutung unter laufender ASS-Therapie werden die Initiation einer PPI-Dauermedikation und die Untersuchung auf H. pylori mit nachfolgender Eradikationstherapie bei positivem Test empfohlen. Eine generelle Testung auf H. pylori vor einer geplanten ASS-Dauermedikation wird in der deutschen S 3-Leitlinie als nicht erforderlich angesehen. Der amerikanische Konsensusbericht empfiehlt einen Test auf H. pylori vor einer plättchenhemmden Therapie bei Patienten mit einem Ulkus in der Vorgeschichte.

Vor dem Hintergrund verschiedener Literaturberichte über die Abschwächung der Plättchenhemmung durch Clopidogrel bei gleichzeitiger PPI-Einnahme brachte die FDA im Januar 2009 eine Sicherheitsprüfung (Ongoing Safety Review) auf den Weg [4]. Gleichzeitig riet die US-Behörde den Ärzten, an der Verordnung von Clopidogrel festzuhalten und die Notwendigkeit für den Beginn oder die Fortsetzung einer begleitenden PPI-Therapie zu überprüfen. Die FDA sah sich zu diesem Zeitpunkt zu einer endgültigen Einschätzung der Problematik noch nicht in der Lage und sprach demzufolge keine strengeren Restriktionen aus. Die europäische Arzneimittelagentur (EMEA) ging weiter und warnte vor der simultanen Einnahme von Clopidogrel und einem PPI [5]. Konkret formulierte sie, dass diese Kombination nur noch dann Anwendung finden sollte, wenn dies absolut notwendig ist. Zwischenzeitlich hat auch die FDA der Standardinformation von Clopidogrel eine Warnung vor der gleichzeitigen Einnahme von Cimetidin und verschiedener PPIs hinzugefügt. Betroffen davon sind Omeprazol und Esomeprazol [6].

Angesichts der aktuellen Datenlage, die in den bislang vorliegenden Leitlinien und Empfehlungen nicht berücksichtigt ist, sahen die DGVS und die DGK die Notwendigkeit, eine gemeinsame Stellungnahme zum Einsatz von Clopidogrel/Plättchenhemmern und PPIs zu erstellen.
