Die retrograde venöse Drainage beim Gefäßersatzshunt – Ein Beitrag zu Shunthämodynamik und Shuntremodeling

Eine retrograde venöse Drainage beim Prothesenshunt kann eine verminderte oder fehlende antegrade Drainage kompensieren oder ersetzen. Kommt es zu einer therapiepflichtigen Armschwellung, kann die retrograd drainierende Vene oft zur autologen antegraden Rekonstruktion verwendet werden. Aus diesem Grunde sollte bei der prothetovenösen Anastomose beim Gefäßersatzshunt die End-zu-Seit-Anastomose favorisiert und die Vene nach distal nicht unterbunden werden.

A retrograde venous drainage can compensate or replace a reduced or failing antegrade drainage of a graft-shunt following a stenosis or occlusion of the proximal draining vein. In case of a severe swelling of the arm, the hypertroph dilated retrograde draining vein can be used for the autologeous reconstruction of the antegrade drainage. Therefore the venous anastomosis of the graft-shunt should be performed as an end-to-side anastomosis without ligating the distal vein.
