Abstract

Symptome wie Kurzatmigkeit und leichte Ermüdbarkeit und häufige Hospitalisierungen verschlechtern die Lebensqualität und Prognose von Patienten mit chronischer Herzinsuffizienz (CHI). Da eine Eisenmangelanämie mit einem schlechten klinischen Zustand der Patienten und einer ungünstigen Prognose korreliert, ist man in der FAIR-HF-Studie der Frage nachgegangen, ob eine Korrektur des Eisendefizits die funktionelle Kapazität und die Lebensqualität der betroffenen CHI-Patienten verbessern kann. Bei Patienten mit symptomatischer CHI (NYHA-Klasse II/III) und systolischer Dysfunktion wurde Eisenmangel diagnostiziert (Ferritin-Spiegel < 100 µg/L; oder Ferritin 100 – 299 µg/L plus Transferrinsättigung < 20 %) und Therapie mit i. v. Eisencarboxymaltose im Vergleich zu Plazebo für 24 Wochen durchgeführt. Die Therapie hat schnell und hochsignifikant die Symptome, Leistungsfähigkeit und Lebensqualität verbessert. Die Therapie war gleichermaßen bei Patienten mit und ohne Anämie erfolgreich und sie wurde gut vertragen.
