Anastomoseninsuffizienzen im Gastrointestinaltrakt sind oft schwerwiegende Komplikationen. Patienten mit einer Beeinträchtigung ihres klinischen Zustands durch die Insuffizienz und deren Folgen wie Mediastinitis, intraabdominale Flüssigkeitsansammlung, Abszess, Peritonitis oder Sepsis, aber auch asymptomatische Patienten mit einer Sentinel-Blutung bei Pankreasfistel bedürfen in der Regel einer intensivmedizinischen Überwachung oder Therapie. Entscheidend ist der Verdacht auf eine Anastomoseninsuffizienz bei jeder klinischen Verschlechterung des Patienten und die frühzeitige, gezielte Diagnostik.

Bis zum Ausschluss sind die gastrointestinalen Anastomosen bei postoperativer klinischer Verschlechterung des Patienten oder bei ansteigenden Infektionsparametern verdächtig.

Die kontrastmittelverstärkte CT nimmt einen wichtigen Stellenwert bei der Diagnose und zunehmend auch bei der Akuttherapie ein (CT-gesteuerte interventionelle Drainage). Die interventionellen endoskopischen und angiografischen Verfahren können heute weitere Komplikationen der Anastomoseninsuffizienzen zuverlässig abwenden und oft zur definitiven Therapie erfolgreich eingesetzt werden (z. B. endoskopisches Einbringen eines Stents über eine Ösophagogastrostomie, Schienung einer Leckage der Gallengangsanastomose mit der ERCP oder das Einführen eines Gefäßstents bei Arrosionsblutung). Eine chirurgische Revision ist bei Dünndarm-Anastomoseninsuffizienz, ausgeprägter Nekrose der Anastomosenenden oder zur Passageausschaltung (Anlage eines protektiven Stomas) in der Regel indiziert. Die Indikation zur chirurgischen Revision hängt dabei vom postoperativen Zeitpunkt sowie dem klinischen Zustand des Patienten ab.

Die mittlerweile zur Verfügung stehenden Konsensusdefinitionen für Anastomoseninsuffizienzen nach Pankreas-, Gallengangs- und Rektumanastomosen sollten den behandelten Ärzten bekannt sein und standardmäßig verwendet werden, um in Zukunft eine transparente Verbesserung unserer medizinischen Behandlung erzielen und bewerten zu können.
