Peritonealdialyse-Katheter-Implantation – Die Sicht des Nephrologen

Mit der Katheterimplantation werden die Weichen für spätere katheterassoziierte Probleme gestellt. Die Mindestanforderung ist ein Katheterüberleben von mehr als 80 % nach 2 Jahren. Entscheidend für den Erfolg ist, das Kathetermodell, die Implantationstechnik, die Wahl der Austrittsstelle und den Verbandswechsel aufeinander abzustimmen. Bislang stellte sich kein Kathetermodell bzw. keine Implantationstechnik als überlegen heraus. Die Erfahrung des Chirurgen scheint eine bedeutende Rolle zu spielen. Der Trend geht in Richtung laparoskopische Katheterimplantation, wobei diese Technik noch einer großen Variationsbreite unterliegt und bisher nicht standardisiert ist. Antibiotikaprophylaxe wird empfohlen.

Correct catheter implantation is the basis to avoid catheter-associated problems. The survival of the catheter should be at least 80 % after 2 years. It is of great importance for the success of the catheter implantation that the model of the catheter, the implantation technique and the way of dressing the exit fit all together. The experience of the surgeon in catheter implantation has a great impact on the success. There is a trend towards more laparoscopic implantation, but this method is not standardized. Antibiotic prophylaxis is recommended.
