Kataraktchirurgie und altersbedingte Makuladegeneration

Die Katarakt stellt die häufigste Ursache einer reversiblen Sehverschlechterung beim alternden Menschen dar. In Deutschland ist die altersbedingte Makuladegeneration (AMD) die häufigste Ursache einer gesetzlich anerkannten hochgradigen Sehbehinderung oder Erblindung. Es werden Fragen erörtert, die sich bei der Behandlung von Patienten mit gleichzeitigem Vorliegen von Katarakt und AMD stellen. Bei adäquater Indikationsstellung profitieren auch Patienten mit altersbedingter Makuladegeneration von einer Kataraktoperation. Der Visusanstieg durch die Operation ist bei der Mehrzahl der Patienten über Jahre nachweisbar. Nach derzeitigem Erkenntnisstand stellt die Kataraktoperation kein Risiko für die Progredienz einer AMD dar. Die Verschlechterung einer AMD nach Kataraktoperation ist ein zufälliges Ereignis, das durch die hohe Komorbidität von Katarakt und AMD bedingt ist. Es gibt kein belastbares Datenmaterial, das den Nutzen der Implantation von Blaufilter-Intraokularlinsen belegt.
