Kryoenergie zur Ablation arrhythmogener paraseptaler Substrate bei Herzrhythmusstörungen im Kindes- und Jugendalter

Zusammenfassung
Hintergrund und Fragestellung: Bei der Kryo-Ablation wird das arrhythmogene Substrat vor der definitiven Ablation zunächst reversibel inhibiert, woraus sich eine erhöhte Prozedursicherheit im Hinblick auf die Vermeidung einer AV-Blockierung bei Ablation in AV-Knoten-Nähe ableitet. Wir berichten über unsere Ergebnisse mit der Kryo-Ablation in einem überregionalem Zentrum für die Ablationsbehandlung im Kindes- und Jugendalter.
Patienten und Methode: Die Verläufe von 39 mit Kryo-Energie behandelten Kindern und Jugendlichen im Alter von 4 – 18 Jahren wurden retrospektiv analysiert. 30 Patienten hatten eine AV-nodale Reentry-Tachykardie (AVNRT), sechs eine parahisäre akzessorische Leitungsbahn (AP), vier eine kongenitale junktional ektope Tachykardie (JET). Neben Nicht-Induzierbarkeit der Tachykardie war der angestrebte Endpunkt bei Ablation einer AP keine oder eine dekrementale konzentrische retrograde Leitung bei Ventrikelstimulation, bei AVNRT das Nicht-Vorhandensein einer „sustained slow pathway conduction“ (SSPC), eines „AH jumps“ oder von Echoschlägen. Die Nachbeobachtungszeit betrug 270 – 1919 Tage, im Median 3 Jahre.
Ergebnisse: Bei 35/39 Patienten (90 %) konnte durch Kryo-Ablation der gewünschte Endpunkt erzielt werden, bei 4/39 Patienten (10 %) musste auf RF-Ablation gewechselt werden. Von den 35 Patienten nach primär erfolgreicher Kryo-Ablation hatten 7/35 (20 %) ein Rezidiv. Zwei Patienten mit akzessorischer Leitungsbahn hatten nach Ablation eine Präexzitation, sind aber frei von Symptomen. Somit sind 28/35 Patienten (80 %) nach Kryo-Ablation bezüglich der behandelten Arrhythmie asymptomatisch, 26/35 (74 %) als geheilt anzusehen. Die Gruppe der Patienten mit AVNRT unterscheidet sich nicht von der gesamten Gruppe. In der Kryo-Gruppe hatte kein Patient eine AV-Blockierung.
Folgerung: Die Kryo-Ablation ist eine effektive und sehr sichere Methode zur definitiven Behandlung von Herzrhythmusstörungen im Kindes- und Jugendalter. Die hohe Prozedursicherheit bezüglich der Vermeidung von AV-Blockierungen wird jedoch mit einer geringeren Erfolgsrate und einer höheren Rezidivrate erkauft.
Abstract
Background and objective: Cryoablation is safe for the ablation of substrates in proximity to the AV node, because the initial lesion is reversible. We report our results of cryoablation in a transregional center for ablation in children and adolescents.
Patients and methods: Data on 39 children and adolescents (4 – 18 years of age) who had been treated with cryo energy were analyzed retrospectively. The diagnosis was AV nodal reentry tachycardia (AVNRT; n = 30), para-Hisian accessory pathway (AP; n = 6) and congenital junctional ectopic tachycardia (JET; n = 4). In addition to non-inducibility, the targeted endpoint for AP-ablation was a missing or decremental concentric retrograde conduction, for ablation of AVNRT the endpoint was no slow pathway, no AH jumps and no echo-beats. The median follow-up was 3 years (270 – 1919 days).
Results: The targeted endpoint was reached in 35/39 patients (90 %), in four patients (10 %) RF energy had to be applied. A recurrence occurred in 7/35 (20 %) successfully treated patients. Two patients had a pre-excitation again after AP ablation, but no symptoms. Thus, 28/35 patients (80 %) remained asymptomatic after cryoablation, and 26/35 (74 %) are definitively cured, regarding all follow-up data. The subgroup of AVNRT patients does not differ from the entire group. There was no AV block in the cryoablation group.
Conclusions: Cryoablation is very safe and effective for the definitive treatment of arrhythmias in children and adolescents. The price for the high safety is a reduced efficacy and a higher recurrence rate.
