Neben den benignen orbitalen Neoplasien existiert eine ebenso große Vielfalt an malignen neoplastischen Prozessen der Orbita. Eingeteilt werden können diese Erkrankungen in primäre orbitale Raumforderungen und sekundär in die Orbita eingewachsene Prozesse, was bei malignen Tumoren naturgemäß deutlich häufiger als bei gutartigen Prozessen auftritt. Schließlich lassen sich systemische Manifestationen von primär außerhalb der Orbita entstandenen Malignomen (Metastasen) abgrenzen.

In vereinzelten Fällen ergibt sich aus dem akuten Auftreten von Symptomen eine Notfallsituation, die schnelles Handeln erfordert. Zu den absoluten Notfällen gehören primär maligne Tumoren wie das Rhabdomyosarkom oder sekundär maligne Läsionen wie Neuroblastommetastasen oder die orbitale Beteiligung bei Leukämie. Bei diesen lebensbedrohenden Erkrankungen kann die unverzügliche Diagnose und Therapie die Überlebenschance des Patienten wesentlich beeinflussen. Ihre Prognose ist häufig kompliziert durch kosmetische und funktionelle Einschränkungen.
