Thoraxsonographie im Notfall – Schmerzen und Beschwerden im Bereich des Thorax

Schmerzen oder Beschwerden im Bereich des Thorax führen häufig zum Aufsuchen einer Praxis oder einer Notaufnahme. Neben kardialen Ursachen (die hier nicht abgehandelt werden) sind eine Vielzahl von Erkrankungen zu bedenken und oft durch eine Notfall-Ultraschalluntersuchung zu klären. Eine entsprechende Therapie einschließlich interventioneller Maßnahmen kann unverzüglich eingeleitet werden. Das Spektrum der Untersuchungsindikation reicht von Pleuraergüssen über den Pneumothorax, die Pleuropneumonie und die Lungenembolie bis zu traumatischen Ereignissen wie Rippenfrakturen, Verletzungen der Thoraxwand oder Lungenkontusionen. Die Bedeutung der Sonographie liegt in ihrer raschen Verfügbarkeit. Die sonographische Untersuchung ist bei vielen Erkrankungen ohne Strahlenbelastung oder Kontraindikationen treffsicher einsetzbar, Voraussetzung ist eine gute Ausbildung.

Pain and discomfort in the thoracic region are frequent reasons for consultations in a medical practice or an emergency ward. Apart from cardiac reasons (here not to be discussed) many diseases have to be considered and explored by emergency ultrasound testing. Therapeutic measures, if necessary with interventional procedures can be started immediately. Indications for an emergency ultrasound testing range from pleural effusion, pneumothorax, pleural pneumonia and pulmonary embolism to traumatic events like rib fractures, trauma of the thoracic wall or lung contusions. The impact of ultrasound testing lies in its rapid availability. Ultrasound testing can unerringly be performed in many diseases without radiation exposure or contraindications - a good apprenticeship provided.
