Zusammenfassung

Der Begriff Spondyloarthritis (SpA) umfasst eine Gruppe rheumatischer Erkrankungen, die neben einer Entzündung des Achsenskelettes (Sakroiliitis, Spondylitis) auch durch eine Entzündung peripherer Gelenke (Arthritis) und Sehnenansätze (Enthesitis) charakterisiert ist. Die SpA wird heute je nach im Vordergrund stehender klinischer Symptomatik zunehmend in prädominant axiale und periphere Formen eingeteilt. Bei den axialen Formen unterscheidet man die ankylosierende Spondylitis (AS) von der nicht-röntgenologischen Form, bei der definitionsgemäß keine strukturellen, d. h. konventionell-radiologisch sichtbaren Veränderungen vorliegen. In diesem Übersichtsartikel werden neben den klinischen Charakteristika vor allem die Diagnostik und Therapie der axialen SpA dargestellt.

Abstract

The characteristic features of spondyloarthritis are inflammation in the axial skeleton (sacroiliitis, spondylitis) and inflammation in peripheral joints (arthritis) and entheses (enthesitis). According to the leading clinical symptom SpA is today differentiated in the either predominant axial or the peripheral form. The axial SpA are further subdivided in ankylosing spondylitis and non-radiographic axial SpA. This subset is characterized by the absence of structural changes on conventional x-rays. In this review we describe the clinical symptoms, the diagnosis and therapy of patients with axial SpA.
